Job Title: Case Worker - Veteran Shelter Program
Pay Rate: $28 - $30/hr
Location: Northport, NY US 11768
Job Summary
We are seeking a dedicated MSW Social Worker to provide direct and indirect services to Veterans in a shelter setting. The Social Worker helps clients improve their quality of life, enhance social functioning, secure permanent housing, and build long-term stability. This role plays a key part in ensuring Veterans access the resources they need to achieve and sustain independent living.
The facility operates 24/7, offering flexibility within a standard Monday–Friday schedule.
Responsibilities
- Complete initial needs assessments and respond to critical needs during intake, including requesting DD214 military records.
- Orient clients to program rules, expectations, confidentiality, and service procedures.
- Ensure clients’ hygiene, daily living needs, and nutritional requirements are met.
- Develop, monitor, and update Independent Living Plans (ILPs); track weekly progress and outcomes.
- Provide referrals to housing, mental health, medical, substance use, and community resources.
- Maintain accurate and complete case records—including ILPs, progress notes, correspondence, HMIS entries, and policy documentation.
- Immediately report critical incidents and update the Program Director on client progress and behavior changes.
- Participate in fire/emergency drills and demonstrate proper use of safety equipment.
- Follow up with maintenance staff regarding repairs or replacements.
- Relocate clients who were inappropriately referred within required timelines.
- Collaborate with the VA liaison to support Veteran goals and stability.
- Participate in rotating weekend on-call coverage and weekday coverage when needed.
- Maintain professional conduct, confidentiality, reliability, and strong communication.
- Engage positively with clients, staff, visitors, volunteers, and community partners.
- Contribute to a supportive, high-quality shelter environment and perform additional duties as assigned.
Requirements
- MSW required
- LMSW or LMHC preferred
- Prior case management experience preferred
- Experience using HMIS or similar data systems preferred
- Experience in residential programs or working with individuals experiencing homelessness strongly preferred
- Experience with mental illness, substance use, or co-occurring disorders desirable
- Strong documentation, communication, and crisis-response skills
- Ability to work independently, prioritize tasks, and maintain confidentiality
