About the Role

Rising Ground’s Health Services team seeks a Nurse Practitioner to provide advanced clinical care to youth and adolescents engaged in our programs. The NP will work within 29i services, which deliver integrated healthcare to young people involved in foster care, residential treatment, and juvenile justice programs—populations with significant and diverse health needs.

What You’ll Do

- Perform health assessments, diagnoses, and treatments across our Bronx and Brooklyn clinics.
- Prescribe medications and develop treatment plans in line with NYS regulations.
- Collaborate with care teams to align medical and behavioral health strategies.
- Promote preventive care and health education for youth and their families.
- Ensure compliance with healthcare regulations and standards.
- Document clinical care accurately in the Electronic Health Record.

    Your Qualifications

    • Education: Master’s or Doctorate degree in Nursing from an accredited Nurse Practitioner program.
    • Licensure: Active New York State Nurse Practitioner license and RN license.
    • Certification: National Board Certification as a Nurse Practitioner in Family, Pediatric, or a relevant specialty.
    • Experience: At least 3 years of clinical experience in child welfare, primary care, clinic, or residential settings.
    • Skills: Strong clinical expertise, diagnostic acumen, and effective communication.
    • Preferred: Experience with Electronic Health Records (EHRs).

    Work Environment:

    Based at the 29i clinic locations in Brooklyn and the Bronx, with regular travel between sites. Work is conducted in clinical settings within program facilities.

    Position Type/Expected Hours of Work:

    35-hour work week

    Additional Requirements:

    • Authorized to work in the U.S.
    • Ability to work in-person in New York City (NY) for at least 3 days or more per week.
    • Ability to travel to other Rising Ground sites (if required)
