Position: Licensed Practical Nurse – LPN Status – Non Exempt
Scope of work: The practice of nursing as a licensed practical nurse is defined as performing tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a registered professional nurse or licensed physician, dentist or other licensed health care provider legally authorized under this title and in accordance with the commissioner's regulations.
Licensed Practical Nurses function by law in a dependent role at the direction of the RN or other select authorized health care providers. Under such direction, Licensed Practical Nurses may administer medications, provide nursing treatments, and gather patient measurements, signs, and symptoms that can be used by the RN in making decisions about the nursing care of specific patients. However, they may not function independent of direction.
Qualifications:
- Licensed Practical Nurse (LPN) with experience with OPWDD Population
- Knowledge of computer systems, such as Excel and Word. Therap a plus
- Ability to safely lift up to 50 pounds.
- Ability to sit, stand, kneel, and walk intermittently and for long periods of time.
- Possession of a clean New York State driver’s license.
- Acceptable Criminal History background check-per NYS Justice Center and Agency Standards
Responsibilities:
- Provides nursing services to Residential and Day Hab Individuals for which you are assigned or as needed to assist with coverage in other In Flight locations.
- Serves as health care member to Interdisciplinary Team.
- Guides direct support professionals in the implementation of a program to ensure high quality health care services.
- Monitors assigned Individuals and recognizes change in health status.
- Identifies health care needs, and plans and implements priority of care and appropriate interventions to meet those needs in accordance with established nursing procedures and regulations.
- Reviewstherapeutic response to nursing interventions
- Ability to fluctuate hours according to needs of all involved.
- Responsibilities to include; but are not limited to:
- Ensuring medications available at all assigned sites.
- Scheduling and assisting with appointments/orders/follow-up documentations.
- Ensuring and coordinating yearly vaccinations for agency individuals
EDUCATION AND EXPERIENCE
- LPN Degree
- 3 years’ experience with the IDD population preferred
SKILLS
Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Math: Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals; ability to compute rate, ratio, and percent and to draw and interpret bar graphs. Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, and percentages.
Reasoning: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical form and deal with several abstract and concrete variables.
CERTIFICATES, LICENSES, REGISTRATIONS
- New York State LPN License
- The employee is required to successfully complete required trainings.
- Valid NYS driver license with driving record that meets the insurer’s standards is required

All staff must pass all NYS Justice Center Background Check Requirements, Drug Test and Physical. In addition, the employee has a duty to report any suspected violations of the law (including any motor vehicle infractions) to the Human Resources Director.
