<h3>Job Description</h3>
Job Title: RN - Case Management
Location: Bronx, NY 10461
Schedule: 9:00 AM-5:00 PM, Mon-Fri
Duration: 8 Weeks with possible extension
Local Pay Range: $65 - $72/Hour.
 
Required Experience:
<ul>
<li>Two years of case/resource management and utilization management are required.</li>
</ul>
 
Duties:
<ul>
<li>Coordinate and manage patient care plans to ensure timely and appropriate services.</li>
<li>Collaborate with interdisciplinary teams to optimize patient outcomes and resource use.</li>
<li>Perform utilization reviews and ensure compliance with insurance and regulatory requirements.</li>
<li>Provide patient education and facilitate discharge planning.</li>
<li>Monitor and evaluate care plans, adjusting based on patient progress and needs.</li>
</ul>
 
Education:
· BSN
 
Required Certificates:
<ul>
<li>New York State RN License</li>
<li>AHA BLS</li>
</ul>
