As the Health Home Care Coordinator, you will play a pivotal role in individuals' lives, making a significant impact through:

  • Deliver at least one of the six core health home services to each health home member monthly
  • According to program guidelines, consult with or case conference with member’s providers on progress of care plan services
  • Advocate with appropriate agencies for services and assist members in the scheduling and coordination of services
  • Conduct home visits, as appropriate, and monitor member health status, access to services and quality of received services
  • Gather written materials on chronic illnesses and available community resources to provide to members on a regular basis
  • Maintain case records and documentation to meet NYSDOH, lead Health Home and agency standards

Health Home Care Coordinator Minimum Salary: $23.07

Health Home Care Coordinator Maximum Salary: $23.07

Health Home Care Coordinator Schedule: Monday - Friday 8:30-4:30


Qualifications:

  • Bachelor's Degree in child and family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation, recreation therapy, rehabilitation, social work, sociology, or speech and hearing;
    • OR NYS licensure and current registration as a Registered Nurse and a bachelor’s degree;
    • OR A Bachelor’s level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses;
    • OR A Credentialed Alcoholism and Substance Abuse Counselor (CASAC)
  • Preferred Licensed level clinician with prior experience in a behavioral health clinician or care management supervisory capacity; Working knowledge of the NYS Health Home Program.
  • 2 years (a Master’s degree in a related field may substitute for one year’s experience) either:
    • Providing direct services to persons diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse;
    • OR Linking persons who have been diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse to abroad range of services essential to successfully living in a community setting
  • This position requires a valid driver's license, proof of current car insurance, registration and access to a reliable, working vehicle
