• Knowledgeable of various community resources and partners.

Responsibilities:

1. *Provides case management to individuals to help them understand and overcome personal or social issues *Coordinate counseling and therapeutic services with other professionals such as doctors, nurses, psychiatrists, and social workers.*Conduct consistent follow-up interviews with clients to determine if their needs are met.
2. *Maintain appropriate and timely documentation, progress notes, and screening/assessment tools.
3. *Responsible for caseload updates, data collection, and submission of activities reports.
4. *Ensure that clients are housing ready.
5. *Prepare housing packages.
6. *Ensure that clients are keeping all outside appointments.

Salary: $50,000
