Job DescriptionDescription:

Under the supervision of the Coordinator of Community Habilitation and Family Care programs, the Community Habilitation Specialist is responsible for ensuring delivery of quality services to all individuals served.

Essential Functions:

· Reports to Coordinator of Community Habilitation.

· Ensures that program meet federal and state regulations.

· Monitor and Record individual progress to ensure that goals and objectives are met.

· Completes Res. Hab. notes on a monthly basis according to standard.

· Writes monthly summaries for each individuals served.

· Inspect vehicle prior to use to ensure that they are in safe operating condition.

· Develop individual activities according to Staff Action Plan.

· Responds to all statements of deficiencies issued by the state.

· Participates in pre-intake screening if it would be necessary

· Participate in Life Plan meetings if it would be necessary and complete meeting minutes.

· Provide CH services face to face to assigned individual.

· Responsible to maintains annual trainings update

· All other duties as assigned by the supervisor.

Authority:

This position exercises considerable initiative and independence in setting goals, activities and individual reviews. Under the general supervision of the Coordinator, the specialist will maintain a professional and responsible work environment for the program.

Skills and Abilities:

· Associates Degree or High School diploma/GED

· One year experience working with diverse clients in a human services setting preferred

· Must be bilingual (English/Spanish)

· Valid NYS Driver’s License/Reliable transportation

· A combination of training and experience other than the specified, if judged to be adequate for the job, may be accepted by the President and CEO.
