Job Title: Site Supervisor


Reports to: Licensed Housing Coordinator

Job Category/ EEO: Exempt, FT 12 months, on call, Manager

Department: Residential Services

Remote: No

Pay Range: $60,406


Position Purpose

The Site Supervisor is directly responsible for overall client care and facility maintenance within assigned residence sites. The Site Supervisor has primary responsibility for supervision of direct care staff and for development, implementation, and review of Individual Service Plans.

Reporting Relationships

The Site Supervisor reports directly to the Site Coordinator.

PRINCIPAL DUTIES AND RESPONSIBILITIES

  • Assure that standards of housekeeping, maintenance, health, safety and general appearance of the residence are met; inspect facilities on a regular basis and report necessary repairs and equipment replacement in a timely manner.
  • Prepares for Office of Mental Health site visits, by ensuring that sites and charts are always in audit ready condition.
  • Implement Agency medication administration policy and procedure.
  • Implement fire safety procedures including monthly, unannounced fire drills.
  • Review daily notes on a regular basis.
  • Promptly report any identified problems.
  • Ensures that the programs have the staffing, competencies, and ongoing professional education and development required to serve clients and deliver on contractual obligations.
  • Assure that the rights and dignity of residents are maintained at all times; assure that resident grievances are processed in a timely manner.
  • Orient new residents and complete required paperwork.
  • Develop and implement Individual Service Plans. Regularly and routinely document resident progress toward goals in accordance with Agency policy.
  • Assist with management of resident discharges.
  • Develop and maintain effective working relationships.
  • Assure timely reporting and review of untoward incidents and events.
  • Participate in regular and unplanned meetings.
  • Initiate emergency procedures when indicated.
  • Plan and implement resident activities.
  • Direct menu planning, food shopping and meal preparation, in cooperation with residents.
  • Always ensure necessary and appropriate site coverage.
  • Provide ongoing feedback to staff regarding work performance.
  • Conducts regular staff meetings.
  • Perform other duties as assigned.

KNOWLEDGE AND SKILL REQUIREMENTS

  • High School diploma plus five (5) years' experience in Human Service-related field.
  • Able to demonstrate at least (1) one year of supervisory experience.
  • Evidence of commitment, sensitivity, and creativity in working with and for individuals with handicapping disabilities
  • Experience in developing service plans and conducting assessments.
  • Must have a flexible schedule.
  • NYS Driver's license with evidence of good driving record
  • Must possess use of a reliable automobile.
