Position Title: Nurse Practitioner
Job Location: Hempstead, NY 11550
Employment Type: Full-time/on-site
Key Responsibilities
· Perform comprehensive physical assessments, patient education, and individualized health planning
· Coordinate services across disciplines and with insurance providers to ensure continuity of care
· Advocate for patients by arranging medically necessary services and referrals to specialists
· Collaborate with community resources to secure support for patients in need
· Consult with physicians and healthcare team members to develop and implement care plans
· Participate in LEAN initiatives and support the patient-centered medical home approach
· Maintain continuity of care through regular follow-up and monitoring of patient outcomes
· Engage in quality assurance activities to enhance care delivery
· Provide case management with a focus on health promotion and disease prevention
· Support clinical decision-making in collaboration with nursing staff and administrators
· Serve as a clinical resource and conduct in-service education on practice-related topics
· Perform other duties as assigned to support patient care and clinic operations
Qualifications
· Minimum of 1 year experience as a Nurse Practitioner
· Graduate of an accredited Nurse Practitioner program
· Current licensure as a Nurse Practitioner in New York State
· Basic Life Support (BCLS) certification required
· Specialty certification preferred
· Bilingual in English and Spanish or Creole is highly desirable
