Job DescriptionDescriptionAbout the Role:

We are seeking a detail-oriented, customer-focused, and data-driven Centralized Scheduling & Patient Coordinator to join our team. This role is at the heart of our patient care process—reviewing referrals, scheduling patients quickly, optimizing field staff schedules, and ensuring a seamless experience for patients and referral partners.If you excel in high-energy environments, enjoy problem-solving on the fly, and take pride in delivering top-notch service, this could be the perfect fit.
This role is a hybrid role, with three days in office. The office location is Suffern, NY.
Key Responsibilities

  • Act Fast: Review each referral immediately, ensure it meets our acceptance criteria, and schedule the patient within 1 hour.
  • Optimize Resources: Monitor and adjust field staff schedules daily to maximize patient visits and geographic efficiency.
  • Problem-Solve: Address day-of delays, staffing gaps, or logistical challenges quickly and effectively.
  • Communicate with Impact: Build strong relationships with patients and referral sources through responsive, professional, and persuasive communication.
  • Leverage Data: Use reporting tools to track scheduling times, utilization rates, and identify trends for improvement.
  • Coordinate Logistics: Manage ordering and delivery of supplies for patient visits.
  • Work as a Team: Collaborate in a centralized scheduling pool to support all service areas.



Skills, Knowledge and ExpertiseWhat We’re Looking For:

  • 2+ years of experience in healthcare scheduling, referral coordination, or related operations (home health, hospice, wound care preferred).
  • Strong organizational skills with the ability to juggle multiple priorities under tight deadlines.
  • Comfort with scheduling software, mapping tools, and data reporting systems.
  • Excellent communication skills—both empathetic and persuasive.
  • High sense of urgency and commitment to delivering an outstanding patient experience.
