Job Details
- Evaluate clients' physical functioning in their home environments
- Develop personalized treatment plans with defined goals and visit schedules
- Implement care plans under physician direction and in collaboration with agency staff
- Coordinate with clients, families, and caregivers to support rehabilitation efforts
- Observe, document, and report patient progress and any changes to physicians and team members
- Collaborate with other professionals to establish rehabilitation and maintenance activities
- Provide care across various age groups, with a focus on pediatric populations
Skills You Need
- Current New York State Physical Therapy license and registration
- Minimum one year of PT experience in hospital or long-term care settings
- Pediatric experience required; multi-population experience (adult, geriatric) a plus
- Ability to travel to patient homes using public or private transportation, including in challenging environments
