Position Overview
We are seeking a Board-Certified or Board-Eligible Internist to join a nursing home facility in Brooklyn, NY. This position focuses on providing comprehensive care to geriatric and long-term care patients, working in a supportive and mission-driven environment.
Key Responsibilities
- Evaluate and diagnose patients’ health concerns
- Manage acute and chronic conditions (e.g., hypertension, diabetes, heart disease)
- Prescribe medications and therapies
- Communicate test results and treatment plans with patients
- Monitor and reassess patient progress
- Promote wellness through patient education on diet, hygiene, and disease prevention
- Administer vaccinations and preventive treatments
- Maintain accurate and detailed patient records
- Refer patients to specialists when necessary
- Support long-term care patients with ongoing management and interventions
- Contribute to clinical research and improvement initiatives, if applicable

Requirements
Qualifications
- Education: MD/DO degree from an accredited medical school
- Licensure: Active New York State Medical License
- Certification: Board Certified/Board Eligible in Internal Medicine
- Experience: Experience in geriatrics, nursing home, or long-term care settings preferred
- Technical Skills:
- Proficiency in EHR systems
- Expertise in chronic disease management and preventive care protocols
- Soft Skills:
- Compassionate care approach
- Strong communication and cultural competency skills
