Position Overview

We are seeking a Board-Certified or Board-Eligible Internist to join a nursing home facility in Brooklyn, NY. This position focuses on providing comprehensive care to geriatric and long-term care patients, working in a supportive and mission-driven environment.

Key Responsibilities

  • Evaluate and diagnose patients’ health concerns
  • Manage acute and chronic conditions (e.g., hypertension, diabetes, heart disease)
  • Prescribe medications and therapies
  • Communicate test results and treatment plans with patients
  • Monitor and reassess patient progress
  • Promote wellness through patient education on diet, hygiene, and disease prevention
  • Administer vaccinations and preventive treatments
  • Maintain accurate and detailed patient records
  • Refer patients to specialists when necessary
  • Support long-term care patients with ongoing management and interventions
  • Contribute to clinical research and improvement initiatives, if applicable

Requirements

Qualifications

  • Education: MD/DO degree from an accredited medical school
  • Licensure: Active New York State Medical License
  • Certification: Board Certified/Board Eligible in Internal Medicine
  • Experience: Experience in geriatrics, nursing home, or long-term care settings preferred
  • Technical Skills:
    • Proficiency in EHR systems
    • Expertise in chronic disease management and preventive care protocols
  • Soft Skills:
    • Compassionate care approach
    • Strong communication and cultural competency skills
    • Ability to work collaboratively in a team environment
