Visiting Docs is looking for a compassionate and skilled Nurse Practitioner to provide comprehensive primary care services to residents of assisted living facilities. The ideal candidate will have experience with the geriatric population and/or long-term care, and a commitment to delivering high-quality, patient-centered care.
Essential Duties and Responsibilities
· Conduct comprehensive health assessments of residents, including physical examinations, medical histories, and medication reconciliation.
· Develop and implement individualized care plans in collaboration with the interdisciplinary team, considering the unique needs and preferences of each resident.
· Manage chronic conditions and acute illnesses, providing timely interventions and appropriate referrals as needed.
· Order and interpret diagnostic tests, such as laboratory tests and imaging studies, to aid in diagnosis and treatment planning.
· Prescribe medications and treatments within the scope of practice and in accordance with state regulations and facility policies.
· Participate in care initiation for our chronic care management services.
· Provide education and counseling to residents and their families regarding health maintenance, disease prevention, and self-care strategies.
· Participate in quality improvement initiatives and interdisciplinary meetings to enhance the overall quality of care and resident outcomes.
· Maintain accurate and up-to-date documentation of all assessments, interventions, and communications in the electronic medical record system.
Requirements
· Master’s degree in Nursing and graduate of an accredited Nurse Practitioner program.
· Unrestricted New York State Nurse Practitioner license.
· Unrestricted Drug Enforcement Administration (DEA) permit to prescribe all medications commonly prescribed in the course of the practice.
· Eligible to provide reimbursable services and maintain provider status with Medicare and Medicaid.
Preferred Experience
· A minimum of 3 years of clinical experience, preferably in long-term care.
· Experience with eClinicalWorks (eCW) electronic health records.
Schedule
· Full and part-time opportunities available.
· No evenings or weekends.
· On-site required.
