LPN - Full time, Day or Night shift. 12 hour shifts. Applicable shift differentials for evening and overnight hours.
Provides high quality on-site nursing support to all treatment programs as directed and ensures a safe and therapeutic environment of care.
RESPONSIBILITIES INCLUDE:
- Assist healthcare team with inpatient services including interviewing patients, vital signs, height and weight, data entry into an electronic medical record
- Perform select clinical tests including, but not limited to urine drug screening, pregnancy tests and blood glucose monitoring
- Manage and operate equipment safely and correctly
- Demonstrate infection control procedures and follow universal precautions
- Administer and document medication under physician’s order and follow protocols for safety and reduction of medical errors.
- Participate in the controlled substance counts per protocol
- Assist physicians with office-based procedures such as administration of intramuscular injections
- Observe, record, and report changes in patients’ condition.
- Organize exam rooms, including general cleaning, upkeep, and re-stocking of supplies.
- Field clinically related telephone calls and responds
- Maintain understanding of current treatments, knowledge, and practices in addiction care.
- Work collaboratively with the Director of Nursing and other nursing colleagues to develop personalized treatment plans and model patient-centeredness, delivering care in a culturally sensitive manner
- Support the efforts of the Wellbridge research team to implement program improvement. Manage and promote relationships with academic and research institutions to promote addiction research and training.
- Participate in regular clinical team meetings
- Work closely with the clinical and advocacy teams to coordinate services to patients

ESSENTIAL FUNCTIONS:
Ability to assess medical needs and provide patient care. For patient care needs, this is an on-site role. Consistently move self about patient care areas. Occasionally lift and move medical equipment and supplies up to 50 pounds without assistance. Ability to verbally communicate related to patient care. Consistent computer and phone, general office equipment use. Update medical records while in a stationary position up to 30% of the time.
QUALIFICATIONS:
- Graduate of an accredited LPN program
- Current license to practice as an LPN in New York State.
- Current certificates for BLS and Infection Control
- Clinical competences in phlebotomy and point-of-care (POC) testing
- Experience providing treatment for addiction and co-occurring mental health disorders, stabilization and withdrawal management settings is preferred.
- Familiarity with all relevant healthcare standards: OASAS, ASAM, JCAHO, etc.
- Ability to assess, plan, implement and evaluate nursing care according to individual needs.
- Demonstrate effective communication and interpersonal skills
