The Home Health Aide (HHA)/Personal Care Attendant (PCA 2) at Hilton East Assisted Living Facility is a key member of our care team, responsible for assisting residents with their daily activities and ensuring that medications are administered safely and accurately under the supervision of licensed nursing staff. This role combines clinical support with hands-on care to ensure residents are receiving the highest level of attention, safety, and comfort in accordance with New York State regulations.
Key Responsibilities:1. Resident Care and Assistance:
- Activities of Daily Living (ADLs): Assist residents with bathing, dressing, grooming, toileting, and feeding as needed to maintain their independence and comfort.
- Mobility Support: Help residents with mobility, including transfers from bed to chair, walking, and repositioning to prevent pressure sores and maintain comfort.
- Health Monitoring: Observe and report any changes in residents’ physical or emotional condition to nursing staff, including signs of distress, pain, or discomfort.
- Personal Hygiene and Comfort: Provide support in maintaining personal hygiene, adjusting bedding, providing blankets, or ensuring the resident’s room is comfortable.
2. Safety and Environment:
- Safety Monitoring: Ensure that residents' living environments are safe, free of hazards, and conducive to their well-being. Assist with the proper use of assistive devices (e.g., walkers, wheelchairs).
- Fall Prevention: Be proactive in preventing falls by ensuring that residents' mobility aids are in good working condition and that residents are comfortable and supported during movement.
- Incident Reporting: Report any falls, accidents, or injuries promptly to the supervisor or nurse, and document according to facility policies.
3. Communication and Documentation:
- Resident Records: Maintain accurate and detailed records of care provided, including personal care, medication administration, and health observations.
- Family Communication: Communicate effectively with residents' families regarding their loved one’s condition, medication needs, and any changes in care.
- Team Communication: Work collaboratively with the nursing team, including RNs, LPNs, and other healthcare providers, to ensure a comprehensive, person-centered care plan for each resident.
4. Collaboration with Healthcare Team:
- Care Plans: Assist nursing staff in the development and implementation of individualized care plans for residents, ensuring medication management is part of the care plan.
- Team Meetings: Participate in care team meetings and in-service education sessions to improve skills and knowledge, especially in medication management and resident care protocols.
5. Support to Families and Residents:
- Emotional Support: Provide emotional and psychological support to residents and their families, offering reassurance and assistance with any concerns about medication regimens or care plans.
- Family Education: Educate families on the importance of medication compliance, potential side effects, and how they can assist in supporting their loved one’s care needs at home, if applicable.
Requirements:
Education: High school diploma or GED.
Licensure/Certification:
- Home Health Aide (HHA) or Personal Care Attendant (PCA 2) certification in New York State.
- CPR and First Aid certification (or willingness to obtain).
Experience:
- Prior experience in an assisted living, nursing home, or home care setting preferred.
- Experience in administering medications or assisting with medication management preferred.
Skills:.
- Compassionate, patient, and respectful attitude towards residents and their families.
- Ability to work independently and as part of a team.
- Strong organizational skills and attention to detail, especially in documentation and medication administration.
- Excellent communication skills with residents, families, and the care team.
Preferred:
- Experience in an assisted living or healthcare setting.
- Familiarity with electronic health records (EHR) or medication management systems.
Physical Requirements:
- Ability to lift, move, and assist residents in a safe and respectful manner.
- Ability to stand, walk, bend, kneel, and perform physical tasks throughout the workday.
- Must be able to handle insulin checks/injections.
Work Conditions:
- Full-time and part-time positions available, including day, evening, overnight, and weekend shifts.
- Work is performed in residents' rooms, common areas, and throughout the facility.
- Exposure to various medical conditions and health risks, including potential exposure to infectious diseases.
- Occasional lifting and physical exertion may be required.
