Major Responsibilities
Support and assist individuals with developmental disabilities in building the skills needed to become engaged, contributing members of their community, enabling them to live as independently and successfully as possible.
Detailed Responsibilities
- Promote self-advocacy, informed choice, and independence for all program participants.
- Assist individuals in identifying, pursuing, and achieving their habilitation goals.
- Maintain focus on each participant's personal aspirations as the foundation for all actions and support provided.
- Encourage and facilitate self-determination, community inclusion, and meaningful participation in community life.
- Take all reasonable measures to ensure participants' health, safety, and well-being.
- Schedule, attend, and actively participate in Habilitation Plan meetings at least every six months.
- Provide direct, hands-on services as outlined in each participant's Habilitation Plan.
- Maintain an up-to-date habilitation record for each participant, including ongoing progress notes that reflect daily activities and overall engagement.
- Communicate and collaborate with case managers to address any behavioral concerns or reportable incidents promptly.
- Assist participants with completing all documentation related to the ISS (Individual Support Services) grant.
Qualifications
- Proficiency in computer applications, including Windows and Microsoft Office Suite.
- Knowledge of general office procedures and strong organizational skills.
- Ability to work effectively as part of a team with colleagues, community agencies, schools, and the general public.
- Cultural sensitivity and ability to work respectfully with diverse groups.
- Strong attention to detail and ability to follow directions.
- Excellent oral and written communication skills.
- Required: Valid NY State Driver's License and access to a registered, insured vehicle.
- Required: Current PPD (TB) Test.
Education, Training, and Experience
- High school diploma or equivalent required.
- Associate degree in a health or human services field preferred.
- Minimum of one year of experience working with individuals with developmental disabilities.
