Position Overview:
We are seeking skilled and compassionate Nurse Practitioners to conduct home visits in Suffolk, Staten Island and surrounding areas. This role offers the opportunity to make a meaningful impact on patient care while enjoying a flexible work environment.

Key Responsibilities:

• Conduct comprehensive assessments and evaluations of patients in their homes.
• Develop and implement personalized care plans based on patient needs.
• Manage and stabilize chronic and acute medical conditions.
• Provide high-quality medical care and support to patients and their families.
• Collaborate with other healthcare professionals to ensure optimal patient outcomes.
• Maintain accurate and timely documentation of patient interactions.

Qualifications:

• Valid Nurse Practitioner license in New York State.
• Board certification is required.
• Must be enrolled as a Medicaid provider.
• Previous experience in home healthcare or similar settings preferred.
• Strong clinical assessment and decision-making skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.

Compensation:

• Rate: $100 - $150 per visit, depending on visit type
