Overview

Community Missions, Inc. is seeking a compassionate, skilled Supported Case Manager to provide direct case management services for juvenile probationers and foster care re-entry youth. This role plays a critical part in our restorative justice and youth violence reduction strategy—working closely with families, law enforcement, social services, and the courts to help youth succeed.

You’ll maintain a safe environment, build positive relationships, ensure compliance with regulations, and deliver structured programming to support the personal growth and rehabilitation of at-risk youth.

Key Responsibilities

  • Provide direct case management and counseling to youth and families.

  • Collaborate with police, DSS, probation officers, and court personnel to track progress and address concerns.

  • Facilitate intake procedures, including assessments, drug testing, and referrals to medical/mental health services.

  • Deliver structured programming in life skills, conflict resolution, and restorative justice principles.

  • Maintain accurate case notes, progress reports, and court documentation.

  • Support and monitor youth participation in school, appointments, and community activities.

  • Ensure safe, clean, and compliant meeting spaces and group settings.

  • Carry agency cell phone for 24-hour on-call accessibility.

Qualifications

  • Education: Master’s in Social Work preferred; Bachelor’s in Social Work, Social Sciences, or related field; Associate’s in Human Services minimum.

  • Experience: Minimum 2 years working with at-risk youth; strong knowledge of Juvenile Justice & Family Court processes; crisis intervention and conflict resolution skills.

  • Requirements: Minimum age 21, valid infraction-free NYS Driver’s License, Red Cross certification (provided if needed).

  • Strong communication, documentation, and small group facilitation skills.
