Patient Coordinator

In this role, you'll enhance patient well-being by providing personalized support and ensuring seamless care coordination.

To be successful, you should have:
- A high school diploma or GED is required; one must type at least 45 WPM and possess basic computer skills.
- Proficiency in common office software (e.g., word processing, spreadsheets); ability to learn proprietary systems quickly.
- Solid verbal and written communication skills; must be able to read and interpret professional documents and draft reports.
Your responsibilities as the Patient Coordinator will include:
- Respond to referrals, verify patient demographics and insurance eligibility, and assess the organization's ability to meet patient needs.
- Obtain and track required physician documentation and secure insurance authorizations per policy.
- Engage with patients/caregivers to determine supply needs; communicate with referral sources regarding case status and alternative resources.
- Document all communications, process updates in the system, and ensure timely delivery setup.
- Work with internal teams and external partners to resolve issues, support quality standards, and follow company policies and procedures.
