Nurse Practitioner – Primary Care
Location: Bronx, NY
Position Type: Full-Time
Salary Range: $135,000 – $145,000 annually (commensurate with experience; may include bonuses/incentives)

Position Summary
The Nurse Practitioner (NP) will provide comprehensive primary care services, delivering advanced clinical care to patients in both in-person and telehealth settings. This role includes evaluating, diagnosing, and managing a wide range of acute and chronic conditions while collaborating with multidisciplinary teams to ensure high-quality, coordinated care.
Key Responsibilities
- Conduct comprehensive physical examinations and patient assessments
- Diagnose and treat a wide variety of acute and chronic conditions
- Develop and implement individualized treatment plans
- Order and interpret laboratory tests, imaging studies, and diagnostic procedures
- Prescribe medications and administer treatments as appropriate
- Maintain detailed and accurate patient documentation including medical history, assessments, and care plans
- Provide vaccinations and preventive care services
- Educate patients on disease prevention, diet, hygiene, and overall wellness
- Coordinate referrals to specialists within the network as needed
- Ensure adherence to quality standards and regulatory requirements
Qualifications
- Master’s Degree from an accredited Nurse Practitioner program (Family or Adult-Geriatrics focus)
- Active New York State Nurse Practitioner license
- National Board Certification (AANP, ANCC, or ONCC)
- EMR/EHR experience preferred
- Bilingual Spanish preferred
