Overview:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) with Family Health experience to join our healthcare team. The ideal candidate will provide high-quality nursing care to individuals and families across all age groups, promoting wellness, preventing illness, and supporting ongoing health needs in a family-centered environment.
Key Responsibilities:
- Provide direct nursing care and health education to patients and families in accordance with physician orders and established protocols.
- Assist with patient assessments, administer medications, and perform treatments and procedures as prescribed.
- Support chronic disease management and preventive care, including immunizations, screenings, and wellness visits.
- Collect and document patient histories, vital signs, and observations accurately in the electronic health record (EHR).
- Collaborate with physicians, registered nurses, and other healthcare professionals to develop and implement patient care plans.
- Educate patients and families on health maintenance, disease prevention, and medication management.
- Monitor and report changes in patient condition promptly to the supervising nurse or physician.
- Ensure compliance with infection control, safety, and confidentiality standards.
- Participate in care coordination and follow-up for patients with ongoing family or community health needs.
Qualifications:
- Active LPN License in the state of New York
- Minimum 1–2 years of experience in a family health, community health, or primary care setting preferred.
- Current CPR/BLS certification.
- Strong clinical assessment, communication, and organizational skills.
- Ability to work collaboratively within a multidisciplinary healthcare team.
- Proficient in electronic health records and basic computer applications.
