Job Description

The Family Nurse Practitioner (FNP) provides primary care services to a diverse patient population across the lifespan. The FNP will work within an interdisciplinary team to deliver high-quality, evidence-based care with a focus on prevention, chronic disease management, and health promotion.

The major duties of the position include, but are not limited to:

Direct care

  • Conduct comprehensive health assessments and physical exams for patients of all ages
  • Diagnose and treat acute and chronic illnesses within scope of practice
  • Order and interpret diagnostic tests, labs, and imaging
  • Prescribe medications and manage pharmacological and non-pharmacological treatments
  • Provide health education and counseling to patients and families
  • Provide basic mental health screening and treatment of non-emergency mental health conditions such as anxiety and depression
  • Explain treatment and conditions to patients in an appropriate and understandable manner
  • Participate in night and weekend call rotation
  • Evenings and weekends shifts may be required

Compliance

  • Complete charting in EMR (eClinicalWorks) and required clinical and billing paperwork in accordance with Apicha’s policies
  • Practice medical care within the scope of license, training and credentialing and in accordance with current standards of care and quality criteria adopted by Apicha, NYSDOH and HRSA.
  • Practice safety, environmental, and/or infection control methods.
  • Immediately alert the Director of Medicine of medico-legal risk situations, and work to reduce same.
  • Maintain confidentiality of patient care and agency operations at all times
  • Advise management on performance deficiencies of ancillary medical personnel
  • Comply at all times with applicable laws and FQHC requirements, as well as the rules, regulations, policies, procedures and Medical Staff Bylaws of Apicha as adopted by Apicha from time to time.
  • Participate in Medicare, Medicaid and such other third-party health insurance plans as determined by Apicha from time to time.
  • Assign to Apicha any and all billings and collections for your services and rights to any incentive program payments relating to your services or use of electronic systems.
  • Refrain from providing outside services unless specifically permitted by Apicha in writing
  • Refrain from soliciting Apicha patients or referral sources to another provider

Collaboration

  • Coordinate care with other medical providers, behavioral health clinicians, case managers, and support services
  • Support initiatives that improve access to care, same-day appointments, and population health metrics
  • Collaborate with multidisciplinary team; attend team/clinic/agency meetings
  • Serve as a preceptor or mentor to nursing or NP students, if needed
  • Work with the support and administrative staff in a positive, constructive manner

QA/QI

  • Participate in clinic-wide Quality Improvement program, peer review and chart audits
  • Participate in appropriate maintenance of certification/continuing medical education
  • Participate in patient education/recruitment activities

Qualifications:

  • Graduate of accredited Nurse Practitioner Program required
  • National certification as a Family Nurse Practitioner (AANP or ANCC) required
  • Unrestricted license to practice in NY required
  • Current DEA certificate without restriction required
  • BLS required
  • Minimum 1-2 years of experience in a community health or primary care setting preferred
  • Knowledge of HIV medical care a plus; AAHIVM or HIVMA certification preferred
  • Ability to use common office software (Word, Excel and PowerPoint), and EMR required
  • Understanding of health issues/concerns of residents in Jackson Heights area preferred
  • Must have good human relations and communication skills
  • Fluency in English required. Bilingual is a plus (English and Spanish or South Asian or East Asian or Southeast Asian)
