Scope of Position: The HOME (Helping Organize My Environment) Team Coordinator will oversee the daily operations of an innovative program designed to support adults with behavioral health conditions and hoarding tendencies living in supportive housing. This role involves supervising a multidisciplinary team, including clinicians and peer specialists, to ensure the delivery of effective, client-centered in-home services aimed at reducing clutter, promoting safety, and improving quality of life. The Coordinator will collaborate closely with internal departments—such as property management, facilities, and housing programs—as well as external partners, including researchers and clinical experts, to implement evidence-based interventions like CREST (Cognitive Rehabilitation and Exposure/Sorting Treatment).
Essential Position Functions:
- The HOME (Helping Organize My Environment) Team Clinician will provide in-home clinical care as part of an innovative new team that exclusively serves adults with behavioral health diagnoses and hoarding tendencies living in supportive housing.
- Must be comfortable conducting community-based visits, including to both single-site locations and scattered-site apartments, to help reduce object acquisition/saving and assist with organization to enhance safety and quality of life.
- Will collaborate with Bridge staff across various departments, including facilities, property management, and housing programs, to support clients.
- Will work in partnership with leading experts, including researchers at the University of San Diego, and receive training in CREST (Cognitive Rehabilitation and Exposure/Sorting Treatment).
- Will be supervised by a Licensed Social Worker and eligible to accrue licensure hours.
- Monthly MetroCard provided; no on-call responsibilities.
Requirements:
- Master’s degree in Social Work or Mental Health Counseling required.
- Experience working with adults with mental illness and/or hoarding disorder is a plus.
- Basic computer literacy required, including the ability to use the agency’s Electronic Health Record system to document services and track data.
- Bilingual in Spanish is a plus.
