Job Description
The Behavioral Health Patient Navigator assists individuals in accessing and navigating mental health and substance use treatment services. In collaboration with Medical Providers, the Behavioral Health Patient Navigator ensures patients receive clear guidance, coordinated care, and access to essential resources throughout their healthcare journey. The Patient Navigator also addresses critical barriers to care and health-related social needs, such as poverty, housing instability, rent burden, and lack of health insurance for behavioral health patients.
Experience
- Education – An associate (A.A.) degree with 2 years of experience, or a Bachelor’s degree with 1+ years of experience in a clinical, medical, or mental health setting.
- 1+ years’ of experience working with Behavioral Health and/or Substance Use disorder patients is preferred.
- Proficiency in the use of EMR, experience in medical chart review, including MS Excel, MS Word, PowerPoint, and internet navigational skills; and highly developed interpersonal and communication skills.
- Bilingual Preferred. Proficiency in English and Spanish or Chinese, both written and spoken language.
- Experience working in medical settings and interacting collaboratively with medical teams. Possess a working knowledge of HIPAA regulations; FQHC experience preferred.
- Excellent communication and organizational skills with professional demenor.
