Self-Direction DSP - Yorktown Heights Area (Job #HT/SR)
Exceptional Family Resources is seeking a Community Habilitation Professional to work in the Yorktown Heights, NY area providing community habilitation to a young man whose interests include playing on his tablet, watching tv, and being outdoors in structured environments.
With the support of his family, he is looking for individuals who are reliable, caring, and able to work with someone with limited expressive language skills. The staff would need to be able to work with someone with limited vocabulary on social skills, safety skills, attending classes and clubs, recognizing their own name and address, and utilizing their independent living skills. The family is looking for someone with the ability to encourage interactive activities with the member.
DSP's will also be responsible for providing transportation to and from community outings. Prior experience in the field of developmental disabilities is a plus but not required. High school diploma required. Background check, fingerprinting, and DMV record check will be conducted prior to hire by the non-profit agency, along with general initial training. Specific training to this individual will be provided by the family with ongoing support provided by the agency supervisor. The support broker involved with the person's self-directed services will also be available for assistance when needed.
General responsibilities will also include:
- Follow individual's staff action plan/daily activity schedule.
- Complete all necessary paperwork as needed and in a timely manner to submit for timesheets, reimbursements, and notes
- Must understand and comply with all HIPAA related guidelines
- Complete all required trainings when they are due
Flexibility in scheduling can be requested and will be between the family and the staff; current available shifts are flexible and to be discussed with the family. Evening and weekend availability may be required. The payrate is $32/hr with reimbursement for mileage, and the staff's fees for participation in any activities will be paid for within the self-direction budget limits.
