Job DescriptionGeriatric Physical Therapist

Salary: $75/35 minute session

Locations: In home visits in Brooklyn, Queens and Bronx

Essential Duties and Responsibilities:

We are seeking NYS Licensed Physical Therapists to provide therapeutic services in home settings. Our In-Home Services are provided in the Bronx, Queens and Brooklyn. We utilize a team centered approach fostering collaboration across all disciplines to facilitate the needs of each individual client.

  • Work 1:1 with geriatric patients in their home setting to maintain optimal health and range of motion.
  • Use therapeutic exercises, manual therapy techniques, and specialized equipment to improve patients’ balance, strength, flexibility, and overall mobility.
  • Provide education and guidance to patients and their families regarding the aging process, the importance of physical therapy, and how to safely perform exercises at home.
  • Monitor and document patients’ progress over time, adjusting treatment plans as necessary.
  • Collaborate with families to create programs and work towards goals as directed by the Care Team.
  • Monitor & assess program effectiveness.
  • Document & communicate detailed treatment results & individuals progress to relevant parties.

Education and/or Work Experience Requirements:

  • Master's in Physical Therapy (Required)
  • NYS Physical Therapist License (Required)
  • CPR Certification (Preferred)
  • Prior experience working with the geriatric population
  • Ability to build rapport with clients & their families
  • Passion to help & motivate others
  • Excellent written & verbal communication skills

Physical Requirements:

  • Ability to travel to numerous locations to meet with clients, attend meetings and visit healthcare facilities.
  • Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
  • Must be able to lift and carry up to 35 lbs.
  • Must be able to talk, listen and speak clearly.
