Assistant Clinical Service Director (RN)
Location: New York, NY
Schedule: Full-Time | 8:30 AM – 5:30 PM
Salary: $125,000 – $130,000/year
We’re hiring a Registered Nurse to step into a key leadership role, supporting high-quality, person-centered care for older adults across clinic, home, and day health center settings.
About the Role:
As Assistant Clinical Service Director, you’ll co-lead clinical operations, supervise interdisciplinary teams, and help shape care delivery across multiple settings. This role is ideal for an experienced RN leader with strong operational, communication, and team management skills.
Responsibilities Include:

Oversee clinical and care coordination staff across departments (nursing, rehab, nutrition, social work, home care).

Support personalized care planning and IDT meetings.

Ensure quality standards, compliance, and care delivery across all settings.

Train and mentor clinical staff; monitor performance and documentation.

Address participant concerns and lead service improvement efforts.

Participate in audits, chart reviews, and regulatory readiness.
Qualifications:

Bachelor’s in Nursing (BSN preferred) or related field.

Current RN license in New York State.

Minimum 3 years in a clinical management role (LHCSA, CHHA, or similar).

At least 1 year of experience with elderly or frail populations.

Strong organizational, leadership, and communication skills.

Ability to travel locally and work some weekends.
