POSITION SUMMARY:

The UAS (Uniform Assessment System) Coordinator is responsible for coordinating and supporting the assessment process for clients enrolled in the Managed Long-Term Care (MLTC) program. This position involves a combination of administrative tasks, data management, compliance monitoring, and communication with clients, staff, and healthcare providers.


RESPONSIBLITIES:

  • Scheduling and Coordination: Organizing and scheduling assessments for clients or patients, ensuring timely completion.
  • Data Management: Collecting, entering, and maintaining accurate assessment data in the system.
  • Compliance Monitoring: Ensuring that assessments meet MLTC and organizational standards.
  • Communication: Acting as a liaison between clients, MLTC and nurses to facilitate the assessment process.
  • Training and support: Providing guidance and support to staff on the use of UAS system and related processes.
  • Reporting: Generating reports and summaries based on assessment data for administrators.
  • Payroll: Prepare and submit payroll for nurses/MLTC.



REQUIRED SKILLS/ABILITIES:

  • Excellent verbal and written communication skills.
  • Strong organizational, time-management, and multitasking abilities.
  • High attention to detail and accuracy in data entry and reporting.
  • Ability to work independently and as part of a team.
  • Proficient in Microsoft Office Suite (Word, Excel, Outlook).


PHYSICAL REQUIRMENTS:


  • Ability to sit for extended periods while working at a desk and using a computer.
  • Ability to lift, carry, push, or pull up to 20 pounds on an occasional basis.


EDUCATION & EXPERIENCE:

  • Previous experience in homecare is preferred
  • Previous experience or knowledge of MLTC is preferred.
  • High school diploma or equivalent
  • Proficient in MS Office
