<h3>Job Description</h3>
Job DescriptionWe currently have an opportunity for a Bi-lingual Chinese-speaking Registered Nurse in Brooklyn to make visits to patients' homes. Key Functions:
<ul>
<li>Perform initial assessment of clients for admission to home care, reassessments, and post-hospital discharge visits</li>
<li>Supervise and instruct PCA/HHAs</li>
<li>Review medication</li>
<li>Develop aides care plan, and MD orders in HHA Exchange</li>
<li>Collaborate with medical providers, and other members of the interdisciplinary team</li>
<li>Work closely with coordination department</li>
<li>Performs patient/family education</li>
<li>Discharge planning</li>
</ul>
Qualifications:
<ul>
<li>Current NYS RN License &amp; Registration required</li>
<li>Bilingual English-Chinese required</li>
<li>Home care/experience with adult/geriatric population preferred</li>
<li>Must be willing to travel to clients homes</li>
<li>Ability to use computerized systems</li>
<li>Excellent verbal and written communication skills</li>
<li>Customer service skills are essential</li>
