As a Home Care Physical Therapist, you will be responsible for evaluating the client's level of physical functioning in the home and for working with the client, family, and/or others for the planning, provision, and coordination of physical therapy services as per MD prescription. You'll also work with other professional staff to establish rehabilitation and maintenance goals and activities.

Responsibilities include, but may not be limited to:
- Developing a plan of treatment that encompasses treatment modalities, frequency of visits, expected outcomes and addresses short- and long-term goals.
- Implements the plan of treatment under physician direction and in collaboration with agency professional personnel.
- Observes records and reports to the physician and program personnel the patients' reaction and treatment and any changes.
Required Qualifications
- License and current registration to practice as a Physical Therapist in New York state.
- Graduate from a physical therapy curriculum approved by the American Physical Therapy Association or the Council on Medical Education of the American Medical Association.
- A minimum of 1-year experience in a hospital or long-term care facility as a Physical Therapist
- Experience working with a pediatric population is required. A combination of experience working with pediatric, adult, and geriatric populations is a plus.
- Demonstrated knowledge of Medicare/Medicaid, regulatory, and credentialing criteria and standards.
- Prior experience as a home care Physical Therapist a plus
- Access to reliable transportation and able to make home visits in assigned service area(s)
