We are a Federally Qualified Health Center (FQHC) that provides Article 16 and 28 clinic services at 8 locations across Suffolk County, New York. We are a multi-specialty healthcare provider caring for our community through Primary and Specialty Medical Care, Dental, Behavioral Health, Audiology, Physical Therapy, Occupational Therapy and Speech Language Pathology. We currently provide care to over 7,000 patients and have an annual operating revenue of $22m.

About the Role:
The Endocrinology Nurse Practitioner plays a crucial role in providing specialized care to patients with endocrine disorders, such as diabetes, thyroid diseases, and hormonal imbalances. This position involves conducting comprehensive assessments, developing individualized treatment plans, and managing ongoing patient care to improve health outcomes. The Nurse Practitioner will collaborate with a multidisciplinary team to ensure that patients receive holistic and effective treatment. Additionally, this role includes educating patients and their families about disease management and lifestyle modifications to enhance their quality of life. Ultimately, the Endocrinology Nurse Practitioner is dedicated to promoting patient health and well-being through expert clinical practice and compassionate care.
Minimum Qualifications:
- Master's degree in Nursing with a specialization in Nurse Practitioner.
- Current Nurse Practitioner license in the state of New York.
- Certification in Endocrinology or relevant clinical experience in the field.
Preferred Qualifications:
- Experience working in an endocrinology practice or similar healthcare setting.
- Experience working with the IDD population.
- Additional certifications in diabetes education or hormone therapy management.
- Bilingual skills to effectively communicate with a diverse patient population.
Responsibilities:
- Conduct thorough patient assessments, including medical history and physical examinations.
- Develop and implement individualized treatment plans based on patient needs and evidence-based practices.
- Monitor patient progress and adjust treatment plans as necessary to achieve optimal health outcomes.
- Educate patients and their families on managing endocrine disorders and promoting healthy lifestyle choices.
- Collaborate with physicians and other healthcare professionals to provide comprehensive care.
