Job DescriptionPosition: Bilingual Case Manager
Location: Brooklyn, NY
Type: Full-Time

Job Summary:
We are seeking a compassionate and organized Bilingual Case Manager to support individuals in a residential setting. The ideal candidate will be fluent in Spanish and English, hold a Bachelor’s degree in Human Services or a related field, and possess a strong foundation in psychosocial rehabilitation and group work.

Key Responsibilities:

  • Provide case management services to residents, ensuring individualized plans are developed and monitored

  • Facilitate group sessions and one-on-one support to encourage personal development and community integration

  • Maintain accurate and timely documentation for all client interactions and progress

  • Advocate for residents' access to services such as healthcare, benefits, and employment support

  • Collaborate with a multidisciplinary team to ensure quality care and coordination

  • Participate in crisis intervention and assist with referrals as needed

Requirements:

  • Bachelor’s degree in Human Services or a related field (required)

  • Fluency in both Spanish and English (required)

  • Previous experience in a residential service setting (preferred)

  • Familiarity with psychosocial rehabilitation principles (preferred)

  • Strong oral and written communication skills

  • Excellent group facilitation and interpersonal skills



Pay Details: $55,000.00 to $63,000.00 per year
