Job DescriptionPosition: Bilingual Case Manager
Location: Brooklyn, NY
Type: Full-Time
Job Summary:
We are seeking a compassionate and organized Bilingual Case Manager to support individuals in a residential setting. The ideal candidate will be fluent in Spanish and English, hold a Bachelor’s degree in Human Services or a related field, and possess a strong foundation in psychosocial rehabilitation and group work.
Key Responsibilities:

Provide case management services to residents, ensuring individualized plans are developed and monitored

Facilitate group sessions and one-on-one support to encourage personal development and community integration

Maintain accurate and timely documentation for all client interactions and progress

Advocate for residents' access to services such as healthcare, benefits, and employment support

Collaborate with a multidisciplinary team to ensure quality care and coordination

Participate in crisis intervention and assist with referrals as needed
Requirements:
-
Bachelor’s degree in Human Services or a related field (required)

Fluency in both Spanish and English (required)

Previous experience in a residential service setting (preferred)

Familiarity with psychosocial rehabilitation principles (preferred)

Strong oral and written communication skills

Excellent group facilitation and interpersonal skills
Pay Details: $55,000.00 to $63,000.00 per year
