Overview Provides physical therapy interventions to designated patients under the supervision of a Physical Therapist ("PT"). Assists in monitoring patient progress through observations and follows up with supervising therapist. Works under PT supervision as required under NY State Education law.- Follows established physical therapy plan of care, including physical therapy interventions and patient/caregiver education. Modifies specific treatment procedures in accordance with PT plan of care and Physical Therapy Assistant ("PTA") scope of practice. - Documents all care provided and maintain records per VNS Health policy and standards. - Assists in monitoring patient progress through observation and follows up with Supervising Therapist in accordance with VNSNY policy. - Complies with scope of practice for PTA, including a co-visit with Physical Therapist on all initial PTA visits and keeps PT informed. PT will re-assess patient every 6 visits or 30 days (whichever comes first). - Manages assigned caseload; provides physical therapy services to patients as directed per PT plan of care. - Participates in VNS Health meetings, including but not limited to, interdisciplinary team meetings, in-service and training programs, rehab services meetings and caseload reviews. - Travels to patients' homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient. - Assists patients with therapeutic interventions and activities of daily living, which may require positioning, moving, transferring and lifting patients of varying body sizes and physical conditions, with and without assistance and as appropriate, instructing, demonstrating and verifying capabilities of family members and/or paraprofessionals. Ability to use and assist patient with use durable medical equipment as a component of the plan of care, including but not limited to wheelchairs, patient transfer lifts, ambulatory devices, slings, splints, orthoses, prostheses, and exercise/strengthening equipment. - Provides care in accordance with Agency policies, practices, procedures and standards, including professional practice standards of care, which may require standing, stooping, sitting, crouching, twisting, reaching, bending and stretching to deliver patient care. - Transports and utilizes required technology devices (tablet), infection control materials and supplies using VNS Health designated/supplied carrying case (backpack) weighing approximately 15 - 20 lbs to and from patient homes/care facilities, VNS Health offices and therapists' home. - Participates in special projects and performs other duties as assigned. Physical Therapist Assistant

Job Summary: The primary purpose of this position is to provide Speech-Language Pathology services for disorders of speech, voice, swallowing and language; plans appropriate Speech-language Pathology services; provides treatment in accordance with accepted professional standards; and participates in the general functioning of the rehabilitation therapy department.
DUTIES AND RESPONSIBILITIES:
Demonstrates Competency in the Following Areas:
Demonstrates working knowledge of skilled restorative therapy and maintenance programs 
Screens residents for rehabilitation therapy needs 
Completes Speech-Language Pathology evaluations upon physician order 
Establishes Speech-Language Pathology plan of care for treatments per physician order 
Provides Speech-Language Pathology plan of treatments and modalities 
Modifies treatment program level, intensity and techniques based on the resident's response to specific treatment plan 
Documents resident progress toward Speech-Language Pathology goals 
Discharges residents from Speech-Language Pathology services when appropriate 
Completes Daily Activity Schedule, progress notes and re-certifications per facility policy. 
Supervises unlicensed assistive personnel 
Provides resident, staff and family education and in-servicing 
Demonstrates working knowledge of Medicare Part A PPS case management 
Complies with Federal and State regulations related to rehabilitation therapy 
Completes all documentation to comply with professional standard and regulatory requirements 
Strives to meet productivity / efficiency goals and self-initiates corrective action plan for non-attainment 
Utilizes the clinical software applications per policies and procedures 
Completes care plans and MDS as directed, per facility policy

Regulatory Requirements:
Must possess a current unencumbered NYS license as a Speech-Language Pathologist or Clinical Fellow with appropriate supervision from a certified and licensed Speech-Language Pathologist.

Valid ASHA certification in Speech-Language Pathology or participating in Clinical Fellow Year Program
Language Skills:

What You'll Do: 
- Collects and records patient care data that follows format and guidelines of the patient care service, regulatory agencies and third-party payers which reflects an understanding of reimbursement systems and their impact on patient care. 
o This includes, but is not limited to, initial evaluation, daily notes, reassessments/progress reports, discharge plans and communication with members of the team. 
- Additionally records daily billing and follows department guidelines for productivity. 
- Prepares for the evaluation in accordance with department policies/procedures and standards of practice including obtaining patient history, exhibiting knowledge of patient diagnoses and researching unfamiliar diagnoses. 
- Selects and administers tests and measures. 
- Additionally, synthesizes evaluation results to formulate the diagnosis, goals, prognosis and plan of care. 
- Establishes treatment plan based on evaluation results, goals and best practice. Collaborates with interdisciplinary team, patients and patient's support system. Assesses and re-assesses need for modifications to plan of care and goals. 
- Identifies need for consultations and/or refers the patient to another health care professional or health care services. Initiates and completes discharge plan. 
- Provides education/training to patient and/or patient support system 
- Training-This includes but is not limited to ADLs/IADLs, therapeutic activities (e.g. transfers, bed mobility), rehabilitation equipment prescription and training and community reintegration 
- Responsible for treatment, plan of care, exercise prescription for patient caseload 
- Provides interventions to patients that are safe, effective, and in compliance with JAG 
- Musculoskeletal Interventions-This includes but is not limited to therapeutic exercise, joint mobilization and modalities 
- Provide data, note writing using Prompt EMR for all treatments provided 
- Oversee support staff of PTA, ATC, PT aides 
- Attends annual JAG Billing and Coding seminar 
- Promotes JAG in the community through lectures/presentations
