Scope of Position: The Licensed Mental Health Professional will play a pivotal role on one of the Governor’s newly launched innovative Safe Options Support (SOS) teams, that will provide comprehensive care to street homeless or subway dwelling individuals. The role will require field-based work, periodic on call coverage, and a willingness to work flexible hours.
 
Essential Position Functions:

<li>Conduct persistent and assertive outreach and engagement using strength-based approaches, beginning at known “hang-outs” or “hot spots” within the transit system or during inpatient hospital admissions or emergency department visits.</li>
<li>Partner and collaborate with current street outreach teams, local police precincts, local hospitals, the MTA, the Department of Homeless Services, and family members/caregivers to identify individuals most in need of outreach and care.</li>
<li>Continuously assess the health and social needs of participants through SOS’s conversational and observational assessments, as well as formalized risk assessment tools for those identified as high-risk.</li>
<li>Collaborate with the centralized SOR Hub to identify available housing and support participants throughout the process, including completing HRA 2010e forms, applying for housing, prepping for interviews, following up with housing providers, and assisting with moving in (day of move), obtaining housing supplies, and learning the neighborhood.</li>
<li>Participate in hospital discharge planning meetings to identify the best community resources for returning participants.</li>
<li>Provide short-term therapeutic counseling and support to participants both pre- and post-housing.</li>
<li>Collect and report data as required, and collaborate with the team leader, data analyst, and other SOS teams to use data to inform future care delivery.</li>
<li>Once participants are housed, work with them and their housing providers to resolve clinical issues impacting the participant’s ability to manage and retain supportive housing.</li>

· Conduct comprehensive assessments with all clients and refer children/individuals/families to supportive and concrete services such as housing assistance, financial assistance, education, substance use, mental health, medical, immigration, legal, etc.
· Conduct assessments within 30 days of intake and review assessments with clients every three months after intake until goals are met, and the case is closed.
· Provide supportive counseling, case management, and crisis intervention. Work collaboratively with survivors to define case goals, objectives, and action plans.
· Meet program expectations for client contacts. Will contact client within 24 hours due to the critical nature of the work.
· Advocate on behalf of clients to overcome systemic barriers related to court, immigration, housing, public benefits, etc.
· Facilitate/co-facilitate groups for clients.
· Develop content, schedule, and participate in monthly outreach events and document attendance of participants.
· Collaborate with internal and external partners to advocate for survivors in court matters, housing matters, public assistance, immigration, etc.
· Complete all data collecting and reporting promptly. Document and maintain all required tracking logs, excel sheets, electronic data systems, and progress notes.
· Participate in meetings, assist in planning for client recreational activities and attend activities.
· Participate in weekly supervision with a planned agenda related to case issues.
· Implement Quality Assurance data to drive case practice, improve performance, and achieve outcomes.
· Participate in program development and agency required trainings on trauma-informed care, DBT, CBT, etc.

Job Summary:
 
The Mental Health Consultant will provide expert guidance, support, and consultation to individuals, families, and staff within the Family Treatment Rehabilitation (FTR) program. This role is central to promoting the emotional well-being of families affected by mental health disorders. The consultant will assess mental health needs, develop intervention strategies, and contribute to creating a supportive, trauma-informed, and culturally responsive environment. The candidate will work directly and collaboratively with internal teams to provide comprehensive assessment, diagnosis, treatment planning, and therapeutic support for individuals experiencing mental health challenges. 
 
 Hours and Schedule
<ul>
<li>Must be available Tuesdays and Wednesdays mornings from 10:00 am - 4:45 pm </li>
<li>Expected hours per week - 15 hours per Month</li>

Requirements:
Skills, Knowledge, and Abilities:
 
• Conducts comprehensive mental health assessments of individuals and families participating in the FTR program and submits documentation within a timely manner.
• Participates in the monthly Clinical Diagnostic Team Meetings and provides expert consultation and case guidance.
• Provides psychoeducation and mental health training or informational support for FTR staff on complex cases 
• Participates and attends mandatory programmatic/agency trainings
• Ensures the integration of culturally responsive and trauma-informed practices into all aspects of care.
• Review internal intake assessments and mental health documentation to support in the identification of mental health concerns.
• Collaborates with FTR staff to develop and implement individualized treatment plans addressing mental health needs.
• Uphold ethical standards, always ensuring client confidentiality and professional conduct.
• Excellent verbal writing, communication and interpersonal skills
• Computer proficiency in Word, PowerPoint, and Excel
• Strong interpersonal skills for engaging with clients, families, and professional teams
• Active listening and the ability to convey empathy and understanding
• Ability to complete all assigned work accurately and within prescribed timeframes
• Culturally competent and sensitive to the needs of various backgrounds
• Adheres to professional codes of ethics and behavioral standards
• Maintains confidentiality and professional boundaries in all interactions

Qualifications/Requirements:
 
• Licensed Mental Health Counselor, or Licensed Clinical Social Worker 
• Bilingual; preferred but not required 
• Minimum of 1 to 2 years of clinical experience working with families, preferably in child welfare, mental health counseling 
• Demonstrate expertise in mental health assessment, diagnosis (DSM-5), and evidence-based therapeutic interventions
• Ability to perform the essential functions of the job with or without reasonable accommodation.
