Position Summary The Licensed Clinical Social Worker provides specialized knowledge and skills for psychiatric and mental health evaluation, assessment and management of patients. They provide crisis intervention, assessments, screenings, individual, family and group counseling services to patients. Essential Responsibilities and Job Duties:

<li>Provides a comprehensive mental health intake/assessment for all new patients referred by primary care provider or other service provider for medical and/or social/emotional issues.</li>
<li>Functions as a patient advocate by participating in clinical meetings, patient care conferences and other discussions pertaining to patients care with medical and mental health program staff.</li>
<li>Assesses the multidimensional needs of patients and develops and implements care plans.</li>
<li>Refers those in need of other or more intensive types of mental health and/or case management services to appropriate sources.</li>
<li>Provides individual, group and family counseling to patients and their families.</li>
<li>Provides crisis intervention/emergency evaluations to patients when required.</li>

Sun River Health is currently seeking a full-time A31 Clinical Program Director at The HUB Health Center. Position Summary The A31 Clinical Program Director provides leadership and acts as a role model for all personnel in the planning and delivery of client services. He/she directs program operations and collaborates with all departments to develop, deliver, and evaluate the clinical program services provided to the Sun River Health clients. The Program Director directly supervises all administrative and case management staff and collaboratively supervises all facilities and plant operations at the program site. This position ensures that concrete services are provided to clients in accordance with New York State Office of Mental Health requirements as well as agency policies, procedures, and quality standards.
