SUMMARY
Provide on-going training and personal assistance to Community Habilitation participants with disabilities with the intent of promoting independence and community inclusion. Work with minimal supervision, provide training, role modeling, structured routines and specialized assistance as outlined in each participant’s habilitation plan. As much as possible, routines will be individualized, based on the participant’s choice of activities and aimed at empowerment and inclusion in the community.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES
The duties and responsibilities of the Community Habilitation Specialist will include but are not limited to the following:
<ul>
<li>Adhere to ethical principles including the agency’s mission statement and the standards of conduct.</li>
<li>Provide support to participant in choosing and participating in a variety of meaningful recreational, vocational, and daily living skills activities based on participant desires, parameters of the program, and their individualized community habilitation plan.</li>
<li>Assess the interests, abilities, strengths and limitations of the people we support.</li>
<li>Follow dietary guidelines as directed in habilitation plan.</li>
<li>Provide personal care consistent with participants skill level and as outlined in the habilitation plan (including bathing, grooming, toileting, dressing, laundry, etc.).</li>
<li>Demonstrate sensitivity and responsiveness to the people we support.</li>
<li>Work with supervisors and co-workers to meet program needs.</li>
<li>Develop an appropriate rapport and work effectively with the circle of support of the people we support.</li>
<li>Keep written and verbal information shared with team members regarding a participant confidential in accordance with HIPPA regulations.</li>
<li>Work with other service providers within the community in a professional manner.</li>
<li>Participate in regular staff meetings and required trainings.</li>
<li>Carry out responsibilities as assigned by the program manager (i.e., doctor’s appointments, religious observances, banking, budgeting, session notes, goal plans, etc.).</li>
<li>Utilize pro-active approach to avert crisis situations.</li>
<li>Utilize on-call supervisor to receive instruction and report emergency situations.</li>

The Patient Service Representative provides customer service to all customers, both over the phone and at the check-in area, for all arriving and departing patients. This position is responsible for scheduling future appointments both in person and via the telephone, registering new patients in the EHR, checks registration forms for completeness and accuracy, and scans registration into patient EHR.
 
Job Responsibilities:
<ul>
<li>Accurately register new patients in the OOH electronic medical record (EMR) system, checking to see if the patient was previously registered and verify demographic and insurance information for accuracy.</li>
<li>Promptly and courteously, greet arriving patients and guests.</li>
<li>Promptly check patient in, including, but not limited to accurately updating insurance, yearly demographic information (registration form) and collecting any payments required from the patient. Promptly check patient out.</li>
<li>Process payments received via mail, phone, or in-person.</li>
<li>Schedules future appointments as needed, at both checkout window and over the phone.</li>
<li>Refer patients to Patient Accounts Office if financial plans are necessary.</li>
<li>Scans all completed patient paperwork into patient EMR in a timely manner.</li>
<li>Maintain phone etiquette; answer phones, transfer calls, take accurate messages and assign appropriately, document prescription refills and assign to correct provider.</li>
<li>Monitors appointment schedules</li>
<li>Respond to Patient/Physician inquiries</li>
<li>Reschedule patient appointments when providers call in sick or schedule time off.</li>
<li>Direct patients that need assistance with their health insurance to the embedded Facilitated Insurance Enroller.</li>
<li>Call appointment reminders for physicals and all new patient appointments and change appointment status to “confirm” when done. Call failed appointment reminder calls to attempt to confirm appointment.</li>
<li>The ability to communicate effectively with patients and staff from all departments.</li>
<li>Attend regular staff meetings, trainings, and other meetings as requested.</li>
<li>Maintain a safe, clean and orderly work area at the front desk and waiting room.</li>
<li>Schedule interpreters (internal or external) for patient appointments.</li>
<li>Balance cash receipts at the end of shift and prepare appropriate reports.</li>
<li>Make sure the building is unlocked in the morning, and locked at night with the alarm set by the last employee to leave the building.</li>
<li>Adheres to all organizational policies and procedures.</li>
<li>Any other reasonable requests from management.</li>

Education/Experience:

<li>1-2 years of medical office experience/call center preferred</li>
<li>1-2 years of prior administrative or customer service experience</li>

Requirements:
Job Requirements
<ul>
<li>NYS licensed and currently registered as a Registered Professional Nurse.</li>
<li>Thorough knowledge of the scope of practice of professional and paraprofessional healthcare.</li>
<li>Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.</li>
<li>Ability to speak effectively before customers/patients or employees of the agency.</li>
<li>Excellent organizational writing, communication and interpersonal skills.</li>

<li>Must have at least 1 year Home Care experience</li>

REGISTERED NURSE (RN) DUTIES/RESPONSIBILITIES:

<li>Provides medical oversight for clients in their homes- including but not limited to assessments, obtaining MD orders, monthly contacts, and maintaining an appropriate Plan of Care</li>
<li>Communicate with referrers, discharge planners, and physicians to ensure seamless patient care.</li>
<li>Develop, implement, and monitor individualized treatment plans, ensuring proper care coordination and medication management.</li>
<li>Perform in-home patient assessments, reassessments, and provide intermittent nursing services.</li>
<li>Ensure accurate documentation and compliance with agency policies, including patient rights and discharge planning.</li>
<li>Supervise and provide in-service education for staff, conducting competency evaluations.</li>
<li>Participate in quality improvement programs and policy reviews.</li>
<li>Instruct PCA Training Program as needed.</li>
</ul>
