Our flagship Ready, Willing &amp; Able (RWA) program provides paid transitional work experience, transitional housing, advanced occupational training, and comprehensive supportive services to individuals experiencing homelessness. The Doe Fund is also one of New York City’s largest nonprofit developers of permanent affordable and supportive housing, with more than 1,900 units in operation or pre-development. The Doe Fund has an annual operating budget of $80 million and a full-time staff of nearly 500, a significant portion of whom are graduates of our programs. 
We are seeking a Mental Health Counselor for the Villa residence which is The Doe Fund's newest permanent supportive housing program for homeless men and women living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. Forty-one units will be allocated for residents referred by the New York City HIV/AIDS Services Administration (HASA) and twenty-seven apartments will be for adults referred by the New York City Housing Authority (NYCHA).
The Mental Health Counselor is responsible for providing supportive case management services to the HASA-funded residents. The successful candidate will have strong clinical and communication skills along with an understanding of working with the HIV/AIDS population.

<li> Analyzes the patients health status, goals and dietary restrictions and creates a suitable nutritional plan for individual patients considering their life circumstances and chronic health conditions.</li>
<li> Tracks patients dietary intake and progress.</li>
<li> Educates patients and families on how to maintain a balanced and healthy diet.</li>
<li> Connects patients with other needed health services such as fitness training and behavioral health counseling.</li>
<li> Promotes nutrition through community wide presentations and awareness programs.</li>
<li> Stays up to date on the latest nutrition research.</li>
<li> Provides nutrition consultation to professional staff and other Seneca Nation programs. </li>
<li> Cooperates with and initiates plans with appropriate community agencies for provision of food assistance for families.</li>
<li> Prepares reports and summaries as required.</li>
<li> Maintains effective verbal and written communications and positive public relations.</li>
<li> Performs duties with sensitivity to community needs and cultural differences.</li>
<li> Actively participates in case conferences and planning which may involve other health care providers.</li>
<li> Follows all policies and procedures of the department, Seneca Nation, and SNHS.</li>
<li> Works to promote cooperative staff efforts to provide and maintain high quality service to patients, physicians and other staff to assure quality care.</li>
</ol>

KNOWLEDGE, SKILLS, &amp; ABILITIES:
 
<ul>
<li> Working knowledge of HIPAA regulations and health informatics.</li>
<li> Good decision making and judgement capabilities.</li>
<li> Ability to communicate with all patients and family members.</li>
<li> Knowledge about diabetes and the nutritional role in the treatment of diabetes and prevention of complications.</li>
<li> Knowledge of the Seneca Nation culture and traditions and the role of nutrition in the culture.</li>
</ul>

 
 
QUALIFICATIONS:
 
<ul>
<li> Masters degree in Nutrition, Dietetics or related field is required.</li>
<li> Registered Dietician Nutritionist Certificate, RDN, from the Commission of Dietetic Registration (CDR)</li>
<li> Must have or obtain Certified Diabetes Care and Education Specialist (CDCES) certification within 5 years of employment.</li>
<li> Public health experience preferred.</li>
<li> Experience in a large hospital, academic medical center, outpatient health care setting, or Indian Health Service (IHS)/tribal health, preferred.</li>
