SUMMARY: 
As a Licensed Practical Nurse, you will provide professional patient care to long-term and post-acute care residents. You will direct and monitor the care provided by CNAs and consistently ensure proper documentation and recording is taking place. Your ability to lead is crucial in both the success of the CNAs and the care of the patients.
Attached to Clifton Springs Hospital, Clifton Springs Nursing Home offers a uniquely holistic approach to long-term care. Skilled nursing care is located adjacent to Spa Springs, where patients can access massage therapy, acupuncture and similar treatments. Clifton Springs Nursing Home is a 108-bed facility.
RESPONSIBILITIES:
· Responsible for timely, accurate medication administration of assigned team and concurrent required documentation.
· Performs treatments and documents progress of same as per physician orders and facility protocol.
· Assures concise, pertinent and complete resident care documentation using computerized medical record process. Assures the initiation of appropriate care pathway tools.
· Responsible for receiving/giving relevant reports, both written and verbal from/to appropriate staff as per departmental protocols.
· Assures proper monitoring of residents and relevant condition status are reported to charge nurse/supervisor and/or physicians in a timely manner.
· Completes pre-reviews of medication administration records as per facility protocols.
· Completes required MDS documentation and all related observations in a timely manner.
· Supports CNA team through assisting in direct ADL care of residents.
REQUIRED QUALIFICATIONS:
· Graduate from an accredited School of Practical Nursing.
· Current licensure in State of New York.
PREFERRED QUALIFICATIONS:
· One year experience preferred.

RESPONSIBILITIES:
· Provides high-quality, patient-centered, clinical care

ESSENTIAL DUTIES AND RESPONSIBILITIES:

<li>Support the Administrator in ensuring effective staff engagement with families and their children to support trauma informed care.</li>
<li>Support the Administrator in supervision of departmental teams, including administrative, social services, program/early education, maintenance, security, and custodial staff.</li>
<li>Assist in recruitment and work assignments for department staff in collaboration with Directors.</li>
<li>Cover Director and other departmental positions, if such positions become vacant or are temporarily absent.</li>
<li>Conduct daily inspections to ensure management staff is present and the physical premises are in order.</li>
<li>Regularly liaise with Director of Family Services to discuss any challenges that may be impeding families’ access to benefits, employment, or housing.</li>
<li>Report weekly, monthly and/or annually to OTDA, DHS, ACS or HFH central office, and monitor DHS and CARES client tracking system, as requested by the Administrator.</li>
<li>Assist in the creation, development, provision, and assessment of staff orientation, training and development, scheduling, and employment assignments.</li>
<li>Assist in development and maintenance of policies and procedures to ensure quality service provision.</li>
<li>Attend meetings, conferences and seminars as needed in order to remain knowledgeable in the field of homelessness prevention and services; stay up to date on current policies and proposed policies that may affect the operations of Tier II shelter facilities. </li>
<li>Assess facility repair and improvement needs and make them known to the Administrator.</li>
<li>Ensure all general health and safety guidelines are in compliance.</li>
<li>Report any incident or disturbance that may jeopardize the health and safety of residents and staff to the Administrator and Director of Security.</li>
<li>Review purchasing, payroll and petty cash to ensure compliance with procedures.</li>
<li>Remain familiar with the various facility inspections conducted by OTDA and DHS and work with Facility Administrator to ensure all requirements are met.</li>
<li>Work with outside providers to ensure quality service.</li>
<li>Perform other related duties as assigned.</li>

QUALIFICATIONS:
