Bilingual English/Spanish Home Care Staffing/Service Coordinators we have . Our Coordinators are a liaison between our Home Health Aides, Patient/Client and the RN-Home Care Supervisor. As a Service Coordinator you will:
- Identify and assign staff to meet client needs
- Communicate with RN-Home Care Supervisor regarding all new clients, changes in staff assignments and client needs
- Provide general information to the public regarding home care services
- Collect initial intake information from clients
- Ensure employee files are complete and up to date with all required documentation
- Be available on a rotating schedule for On Call evening and weekend service
Successful Candidates will have the following skills and experience:
- One year of Home Care Coordination Experience
- Bilingual English/Spanish verbal and written skill is required
- Strong verbal and written communication skills
- Basic data entry skills
- High School Diploma/GED required
