BILINGUAL CASE WORKER

We are seeking a qualified Case Worker – Bilingual (Spanish/English) with a strong commitment to social services and older adults to work at several sites (Kew Gardens, Pomonok, Jamaica and Rego Park). Services include case assistance, benefits counseling, assisting with benefit applications and recertifications, providing information about other services & making referrals, registering new participants, and making telephone reassurance calls. The Caseworker will assist the staff at the centers where they are stationed in building a supportive community for older adults through needs and eligibility assessments, participant advocacy and partnership building.

Primary Responsibilities:

  • Conduct client intake to assess challenges, including personal and family adjustments, finances, and physical or mental impairments, in order to determine nature and severity of the problem. 
  • Provide accurate and appropriate information about available services, benefits and entitlements. 
  • Conduct on-site visits with new program participants to support engagement with other program members. 
  • Develop case notes and care plans, including action items and follow-ups, based on client assessments.  
  • Collaborate with program directors to ensure that client’s presenting needs are addressed using culturally sensitive options that make efficient use of community resources. 
  • Build and maintain partnerships with other agencies to expand resources; advocate and intervene with other agencies on behalf of participants. 
  • Participate in meetings, staff development, and training sessions. 
  • Attend conferences, in-house trainings, and other professional development opportunities.
  • Perform other duties as assigned to support the staff and participants of the older adult centers.

Qualifications:

  • Bachelor's degree in Social Work, Sociology, Human Services, Psychology or related field preferred.  
  • Minimum of two (2) years of experience working with older adults or case management in the senior services sector.  
  • Knowledge of benefits/entitlements and/or case management experience are a plus.  
  • Superior written and verbal communication skills.  
  • Excellent interpersonal skills with demonstrated ability to build and sustain relationships.  
  • Self-motivated and resourceful, able to work independently as well as collaboratively with staff.  
  • Ability to maintain confidentiality and use appropriate discretion.  
  • Proficient in Microsoft Office applications including Word, Excel and Outlook and database management/search tools.  
  • Bilingual-Spanish verbal and written competencies.

Status: Full-Time, Non-Exempt

Salary: $45,000 Annually

Schedule: 35 hours per week, Monday – Friday, 8:30 am – 4:30 pm
