PHS administers WholeYouNYC (WYNYC), a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WYNYC brings together over 100 organizations offering various programs – such as food, housing, employment, health insurance, and sexual health services – across all five boroughs. These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity. To date, our network has already impacted thousands of lives through community partnerships and referrals, generating millions in estimated healthcare savings.
New York State (NYS) recently announced the availability of $500M statewide to support Social Care Network (SCN) lead entities responsible for coordinating social care delivery in various regions across the state. Public Health Solutions (PHS) and our WYNYC network were awarded the role of regional SCN for Brooklyn, Manhattan, and Queens.
This is a grant-funded position ending March 31, 2027.
Position Summary:
We seek an experienced Senior Director to plan, direct, organize and evaluate WholeYouNYC SCN care coordination and delivery. The successful candidate will oversee and provide direction to the WholeYouNYC care delivery teams and organizations supporting the network. The Senior Director is a key contributor to the success of the SCN and works collaborative with external partners (e.g., CBOs, providers, MCOs) and others across the organization including, but not limited to Contract Administration, Finance, Third Party Risk and Compliance and Legal to oversee the day-to-day management of SCN care delivery activities.
Specifically, the Senior Director, WholeYouNYC Social Care Network Care Delivery will:
- Serve as the primary point of contact for all WholeYouNYC SCN care delivery functions and activities.
- Apply best practices and innovative strategies to plan, direct and organize WholeYouNYC SCN care delivery for regional Medicaid members, working collaboratively with CBO and other network ecosystem partners.
- Drive SCN care delivery forward using effective, project management techniques that enable reliable tracking and monitoring of progress over time.
- Initiate and nurture strong relationships with network partners – social services, healthcare, government, technology CBOS, providers, MCOs, government, technology vendors – to foster meaningful and effective partnerships that optimize SCN care delivery and improve the lives of regional Medicaid members.
- Provide social services screening, navigation and delivery thought leadership to increase access to and value of SCN services for regional Medicaid populations.
- Recruit and manage extraordinary, high-performing SCN care delivery teams that can flexibly adapt to the dynamic nature of the work; this includes supervisory duties and performance management to build core competencies of the SCN care delivery workforce.
- Promote team performance and professionalism through mentoring, training, coaching and feedback.
- Ensure full alignment of budget and resources across the various SCN care delivery teams.
- Evaluate SCN care delivery activities, ensuring they are executed in accordance with NY State Department of Health and PHS goals, timeline, budget and quality expectations.
- Work closely with Quality Improvement & Evaluation, other internal and external stakeholders to establish network quality control mechanisms / processes. Continuously strive to enhance SCN care delivery efficiency and effectiveness through process improvement, data driven analysis and performance evaluation.
- Lead and participate in relevant internal and external workgroups, committees and advocacy efforts to elevate the work and impact of the SCN.
- Develop and/or provide input into scopes of work and budgets for SCN care delivery teams including preparing and submitting budget modifications and ensuring network milestones are timely met.
- Provide input into and actively support the development of WYNYC sustainability concepts, proposals and budgets.
- Proactively stay abreast of public health and health equity-related trends, threats and emerging opportunities. Use these learnings to inform SCN care delivery.
- Assist with the creation, development and delivery of compelling presentations to the PHS Board, SCN Governance Committee, executive / senior management teams, other internal and external stakeholders communicating key SCN-related messages.

Qualifications and Experience:
- Bachelor’s degree and/or equivalent required.
- At least 7 years of experience in mid-to-senior level positions in a public health, not-for-profit or consulting setting with responsibility for program development, implementation, management and evaluation.
- At least 3 years of direct project management experience.
- Understanding of the health and human services sector in NYC/NYS and related government agencies.
- Previous experience working directly with government programs including Medicaid.
- Foundational knowledge of the social drivers of health (SDoH) and their impact on health outcomes.
- Excellent team player with the ability to be flexible, work collaboratively both internally and externally and engage in consensus-based decision making.
- Ability to work independently, lead and support growth of program staff.
- Excellent communication skills (listening, written, oral, and public speaking).
- Attention to detail (e.g., to ensure regulatory compliance) with the ability to see the bigger picture for program development.
- Ability to thrive in a work environment that is respectful, goal-oriented, focused, and supportive of work-life balance.

Desired Skills:
- MPH, MPA, MBA or related Master's Degree preferred.
- Project Management Professional certification.
