Job Type: Part-Time (Min 20 Hours) Pay: $20.00-21.00/hour Work Location: In-Person/Community-Based

The Care Management Coalition of Western New York, Inc. (CMC) consists of several non-profit agency partners who share space at 1021 Broadway and collaborate in offering programs and services to the community. We are seeking a highly motivated and dedicated individual to join our team as a Community Outreach Specialist that will report to the Managing Director. The successful candidate will play a vital role in connecting individuals and families in our community to the resources and support they need to thrive. This is an exciting opportunity for someone who is passionate about making a positive impact in the community and has a strong desire to help others.

Responsibilities

  • Provide education, guidance and support to individuals regarding colorectal cancer screening
  • Engage community stakeholders through community events, planned opportunities, or 1:1 meetings to raise awareness of our presence and services offered to the community.
  • Manage inbound calls (inquiries) in a timely manner and route inquiries to the appropriate community resources
  • Inquiries may include calls, face-to-face, drop-in, etc.
  • Greet prospective clients and engage them in understanding their needs
  • Document, log, and report all information related in inquiries in accordance with the program policies and procedures and supervisor guidance
  • Ensure accurate data collection and entry of data.
  • Conduct outreach and engagement activities to connect with the community and bring awareness to community resources
  • Maintain accurate and up-to-date knowledge of community resources

Qualifications

  • High school diploma/GED or equivalent experience
  • Experience working with underserved or marginalized communities
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and organizational skills
  • Strong motivation to address the needs of diverse individuals and families and to improve organizations and systems
  • The ideal candidate can complete all physical requirements with or without reasonable accommodation.

The CMC is a small nonprofit organization with plans for sustainable growth and development. Due to the nature of this, other duties may be assigned not specifically covered in this job description.
