This role ensures care teams deliver high-quality, participant-centered services aligned with best practices, compliance standards, and organizational goals. About the Role:The Care Delivery Manager provides leadership and operational oversight to care teams—including nurses, aides, social workers, and therapy staff—across assigned sites. You will work closely with clinical leaders to ensure care plans are effectively implemented, services are well-coordinated, and participant outcomes are continuously monitored and improved. Key Responsibilities: Manage daily operations of interdisciplinary care teams (IDT) delivering services across home, day centers, and clinical sites Ensure timely execution of care plans and service delivery across disciplines Monitor staffing levels, coordinate team schedules, and ensure coverage for key roles Support staff performance and adherence to clinical protocols, policies, and standards Participate in care planning meetings, clinical reviews, and quality assurance initiatives Act as a liaison between care teams and leadership to resolve barriers to effective care delivery Review operational metrics and participant outcomes; implement corrective actions as needed Facilitate regular team huddles and staff training in support of care excellence and regulatory readiness Support service transitions, including hospital discharges and new enrollments Document and monitor participant-related incidents or grievances in collaboration with compliance teams Engage in audits, reporting, and internal improvement initiatives as assigned Qualifications: Bachelor’s degree in Nursing, Health Administration, or a related field required; advanced degree preferred Licensed RN in NY State preferred 3+ years of supervisory experience in healthcare, preferably in home care, managed long-term care, or a PACE model Experience working with frail, elderly, or chronically ill populations Strong organizational, communication, and critical-thinking skills Ability to manage multiple priorities and lead high-performing teams in a fast-paced environment Proficiency with electronic medical records (EMR) and operational systems Legally authorized to work and practice in NY Medically cleared for direct patient contact; up-to-date on required immunizations Physical Requirements:Must be able to travel between program sites; lift up to 50 lbs; perform prolonged standing, walking, or sedentary tasks; and meet the cognitive, communication, and motor demands of supervising care delivery in clinical and community settings.
