Social Care Navigators | Annual Salary: $50,000 | Full-time | 4 vacancies

Summary: The Social Care Navigator plays a key role in ensuring that the State’s Social Care Network program can reach and deliver health-related social needs (HRSN) services to eligible Medicaid members. We seek an experienced Social Care Navigator to connect vulnerable Medicaid populations living in New York City to needed community-based social supports using an online referral technology platform to track and “close the loop” on referrals. The incumbent will be responsible for engaging with Medicaid members to assess their health-related social needs, confirming eligibility for SCN services and facilitating navigation to needed social supports (prioritizing food, housing and transportation services); all while ensuring access to effective, culturally and linguistically tailored community resources. He/she/they will work independently, but under the supervision of the Social Care Navigator Supervisor to navigate clients to care, share experiences / best practices and troubleshoot issues.

Minimum Education & Experience Requirements:

  • Bachelor’s degree with coursework in community health preferred.
  • 1 - 2 years of experience working in a care navigation / coordination / intake environment especially in human services.

Housing Specialist | Annual Salary: $30,000 | Part-time | 1 vacancy


Summary: The Housing Specialist will play a pivotal role in supporting New Yorkers in the city’s Social Care Network experiencing homelessness as they transition to housing stability.

The incumbent will support the referral and placement process, facilitate connection to appropriate services, conduct assessments, present data that is tracked and coordinate resources. In addition, the Housing Specialist will review determinations and establish relationships with community partners across the City in order meet the housing needs of the participants.


Minimum Education & Experience Requirements:

  • Bachelor’s degree or equivalent certification OR at least 2 years of equivalent comparable housing experience working with economically and socially disadvantaged individuals and/or families.
