Position Summary:
As Assistant Director of Case Management at Carter Burden Network, you will play a pivotal role in leading a dedicated team of Supervisors, Case Managers, Case Aides, and MSW interns. Your focus will be on supporting homebound older adults across a diverse area—from East 59th to East 142nd Streets, stretching from Fifth Avenue to the East River and including Roosevelt Island. You will oversee the intake process, personally guiding the team as they receive and triage referrals, conduct in-depth client assessments, and design comprehensive, personalized care plans. Your leadership will ensure that older adults receive essential services—from home-delivered meals to home care—as well as support accessing benefits and other community resources.

Essential Responsibilities
Working closely with the Director of the Case Management Unit, you will translate operational goals into effective day-to-day workflows. You’ll provide clinical oversight, administrative support, and direct service delivery to ensure the team meets performance standards and regulatory requirements. Your role includes:
Staff Support & Supervision
- Provide weekly or as-needed individual supervision to Case Managers and MSW interns.
- Offer clinical guidance, especially in high-stakes cases or when APS involvement is required.
- Serve as Acting Director when needed.
- Onboard new staff and support SIFI training compliance.
Client Services Oversight
- Supervise intake workflows, assessments, service plans, and referrals.
- Maintain approximately 200 direct case management hours yearly.
- Conduct and support field visits to client homes across the catchment area.
- Ensure emergency protocols and check-in systems safeguard at-risk clients.
Program Quality & Compliance
- Monitor NYC Aging documentation standards.
- Keep abreast of sector trends and share updates with the team.
- Support audits and funder reporting in coordination with senior leadership.
Team & Program Development
- Co-lead weekly staff meetings and case conferences.
- Contribute to policy implementation, program planning, and continuous improvement.
- Provide coverage for intake tasks or case aide responsibilities as needed.
Agency Collaboration
- Attend agency-wide events and champion the CMU.
- Engage with subcontractors to maintain service consistency.
- Assist with internal reporting and data capture for funders.
Required Qualifications:
- LMSW or LCSW with a minimum of 5 years of professional social work experience in aging services or related field and in supervising a team and/or informing clinical best practices
- Advanced interpersonal and case management skills
- Affinity for, and depth of experience in, working with an older adult population
- Completed Seminar in Field Instruction (SIFI) training for supervising MSW students, if candidate does not have completed SIFI at time of hire it will be expected that they complete it within 1 year
- Proven ability to lead case managers and MSW student intern, build individual capacities and promote excellence

Supervisory Responsibilities:
The Assistant Director will guide and motivate a dynamic team that includes Case Managers, Supervising Case Managers, Friendly Visiting Coordinators, the Intake Coordinator, and a Case Aide. As a mid-senior level position, this role typically operates within standard business hours but will occasionally require evening or weekend availability to support programmatic needs, special events, or organizational priorities. Any such out-of-hours commitments will be communicated well in advance and scheduled with respect for your existing availability.
Position Type, Work Schedule
This full-time, exempt role offers a hybrid schedule with flexibility across all CBN locations. Set hours will fall between 9:00 a.m. and 5:00 p.m., Monday through Friday, aligned with program service needs. You must remain fully accessible during scheduled hours via phone and email and be available on-site or in the field as required.
Physical Requirements
This position works in an office building on the UES as well as in the private homes/apartments of our homebound clients.
Mobility and Transportation: The ability to travel to clients' homes within the community, including walking short distances, climbing stairs, and accessing various residential environments that may not be wheelchair accessible. Must be able to drive or use public transportation independently.
Physical Stamina: Ability to carry light supplies and equipment (up to 20 pounds) when visiting clients' homes or community locations. Must also be able to assist with light physical activities, when necessary, such as helping clients with mobility issues during assessments or while transitioning from one area to another.
Office Environment Mobility: Ability to navigate and work within an office building that includes a basement and up to five floors, without elevator access. This requires the capacity to climb stairs multiple times a day while carrying light office supplies or case files, and the endurance to move between floors as needed for meetings or accessing various resources within the building.
Compensation & Benefits
The Assistant Director role offers a competitive salary range between $75,000 and $80,000 per year, in full compliance with the NYC Pay Transparency Act, with final placement based on experience, skills, and internal equity. We offer a generous benefits package that includes multiple health and dental plan options (including 100% employer-paid coverage), 15 vacation days, 4 personal days, 10 sick days, and paid holidays. Additionally, employees are enrolled in a 403(b) retirement plan featuring a 3% employer contribution and the opportunity for a matched contribution after one year.
