JOB TITLE: Team Leader (LCSW – Program Director) FLSA STATUS: Exempt

PROGRAM: Youth Assertive Community Treatment Rockland HOURS: 35 per week

REPORTING RELATIONSHIP: Director of Children’s Clinical Services

SALARY RANGE: $80,000 to $85,000 annually

After six months of employment, you can expect to receive a retention bonus of $5,000.

The Youth ACT Team serves children/youth with Serious Emotional Disturbance (SED) throughout Rockland County, who are returning home from inpatient settings or residential services, at risk of entering such settings, or have not adequately engaged or responded to treatment in more traditional community-based services. Youth ACT team delivers intensive, highly coordinated, individualized services and skilled therapeutic interventions through an integrated, multi-disciplinary team approach to better achieve success and maintain the child in the home, school and community.

SUMMARY of DUTIES

The Team Leader is a full-time staff member, who directs and supervises staff activities, leads team organizational and service planning meetings, provides clinical direction to staff regarding individual cases, conducts side by side contacts with staff and regularly conducts individual supervision meetings. The Team Leader is responsible for direct client services as a member of the professional staff and will provide clinical services to a small caseload. Direct services include person-centered treatment, services that promote recovery, staff supervision as well as overall management of the program. Indirect responsibilities include representing the program at meetings in the community. The Team Leader will support recovery by providing services that reflect our deeply held values that include the recognition of individual rights of self-determination, choice, shared decision-making and collaboration. When possible, the Team Leader will utilize evidence-based practices; always embracing a trauma-informed and person-centered approach.

SPECIFIC DUTIES and RESPONSIBILITIES

  1. Responsible for overall program operations including; recruiting, hiring, training and evaluating of staff, monitoring of billing, revenues and expenditures, compliance with external accountability sources.
  2. Responsible for adhering to all standards and guidelines established by applicable regulatory authorities.
  3. Responsible for program compliance with external accountability resources and the monitoring of case records.
  4. Prepare monthly and annual reports of services, which provides a basis for continued program planning.
  5. Participate in meetings with community agencies and other service providers.
  6. Monitor intakes & discharges. Direct and coordinate clinical admissions.
  7. Provide community-based clinical and recovery-focused services which may include, but are not limited to, assessment and collaborative treatment planning, individual, family and group psychotherapy, crisis counseling, care management, and community consultation and education.
  8. Facilitate daily morning team meetings.
  9. Monitor rotating on-call responsibilities and provide back-up to on-call staff.
  10. Model and teach a person-centered approach, honoring choice, self- determination and shared decision-making.
  11. Direct day-to-day clinical operations.
  12. Coordinate assessment and treatment planning process.
  13. Execute quality assurance activities.
  14. Develop workflows and policies and procedures for the program.
  15. Educate community resources regarding the Youth ACT Team and maintain positive working relationships with community organizations.
  16. Use individual client data to inform clinical decision-making and identify areas for quality improvement; use data to track client progress over time.
  17. Follow the established agency policies and maintains appropriate professional standards.
  18. Establish and maintain a culture that is recovery-oriented and grounded in service excellence.
  19. Provide supervision, mentoring, coaching and performance reviews for direct reports.
  20. Other designated and related duties.

REQUIRED KNOWLEDGE AND SKILLS

  • Effective verbal and written communication skills.
  • Must complete the CANS-NY Training, be certified to use the CANS-NY, and maintain annual certification.
  • Use Motivational interviewing, recovery and trauma-informed approaches when delivering services.
  • Proficient in computers.
  • Effective crisis intervention skills.
  • Effective leadership and problem-solving skills
  • Highly organized, detail oriented, and self-motivated.
  • Familiarity with and ability to use an Electronic Health Record system.
  • Empower those receiving supervision and offer criticism in a constructive manner
  • Understand employment and labor laws

QUALIFICATIONS

  1. Experienced mental health professional with a LCSW or a Licensed Psychologist.
  2. Must have at least 2 years’ experience in supervisory level positions.
  3. Must demonstrate positive leadership, a deep belief in recovery and self-determination, flexibility, patience, and effective written and verbal communication skills.
  4. Able to perform their duties with minimal supervision and consistently use good judgment. Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
  5. Computer skills in Microsoft office, spreadsheets and database management are required.
  6. Valid Driver’s License and a driving record that is satisfactory to our insurance carrier.
  7. Must maintain a current New York State Defensive Driving certification.
  8. Must be cleared by the Justice Center for the Protection of People with Special Needs Staff Exclusion List (SEL), the New York State Medicaid Exclusion List and the NYS Office of Children and Family Services (OCFS) for instances of child abuse and/or neglect.
  9. The Team Leader must be legally eligible to work in the United States.
