Job DescriptionDescription:

Responsible for delivery of varied social work services to HPCI patients and their families. Provides initial psychosocial evaluations, ongoing psychosocial counseling, direct casework services, patient/family education and advocacy, discharge planning, bereavement services, community education, outreach and referral, and participates in the development of the plan of care.

  • Performs comprehensive initial and ongoing psychosocial and bereavement assessments in a timely manner.
  • Provides individual, family, and/or group counseling to the terminally ill, their families and bereaved within expected scope of practice and as needed.
  • Develops, implements, documents and revises the plan of care for psychosocial concerns as needed in a timely manner, in conjunction with theIDG.
  • Maintains Department of Health, HIPAA and other regulatory compliance with documentation, plan of care and delivery of services.
  • Prepares for and participates in IDG, promoting inter-disciplinary collaboration and addresses psychosocial interventions.
  • Develops and maintains contact with appropriate community agencies and has knowledge of community services in order to promote inter-agency cooperation and to facilitate related referrals.
  • Provides education to staff, volunteers, and community on hospice social work role and death and dying issues. Participates in the orientation of new members of the IDG and conducts hospice training on psychosocial issues as needed.
  • Participates in admission/discharge procedures and decisions as appropriate.
  • Assumes the active role of advocate for the patient/ family unit.
  • Submits required information for reports, grants and other relevant processes as requested.
  • Participates in social departmental activities as assigned, including monthly departmental and other relevant meetings.
  • Completes all documentation in accordance with the standards.
  • Completes all educational requirements with the standards.
  • Meets Social Worker visits in accordance with standards.
  • Public speaking in the community as requested.
  • Performs other duties as assigned.

Requirements:

EDUCATION, TRAINING AND EXPERIENCE: Masters’ degree in Social Work from an accredited educational institution. License preferred but will consider someone preparing to sit for licensure. Incumbent will have one year experience in individual, family and group counseling, preferably in a healthcare or hospice setting.


CERTIFICATION/LICENSURE: Possess appropriate certificate / license to practice as a Licensed healthcare professional in the State of New York. Certification in Hospice & Palliative Care preferred if offered for discipline.
