All Staten Island programs are allotted a $4,420 commuter differential that is applicable to anyone who currently works or is hired to work on Staten Island who doesn't live in Staten Island in addition to the base salary or hourly wage.

The Jewish Board's Community Behavioral Health Clinics provide compassionate, high quality, evidence-based services to individuals and families in the communities we serve. Our staff uses a culturally competent, person centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency." Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resilience of our communities.
PROGRAM DESCRIPTION
The clinic is under the Certified Community Behavioral Health Center (CCBHC) umbrella. Its purpose is to provide integrated care to members of the community. The program effectively meets the needs of children, youth, and adults with behavioral health challenges and individuals with co-occurring substance use disorders.
POSITION OVERVIEW:
The Bridges to Care team works directly with individuals with mental illness, substance use disorders, and/or physical health conditions. Work is office based, however, when there is a need, clients will be met in the community when appropriate. The clinician uses a recovery-oriented lens and evidenced based approaches to evaluate participant's strengths and resources, identify barriers to engagement in services, ascertain level of risk and appropriate services to manage risk, assist with managing and keeping appointments, provide psychoeducation and coordinate other aspects of participants' health and community services to support engagement. The clinician also provides crisis services to participants remotely or in person in the office or community.
KEY ESSENTIAL FUNCTIONS (List the functions that are essential to achieve the goals):
Conduct assessments with clients discharged from the hospital, to evaluate participants' strengths and resources, current functioning, barriers to engagement, level of risk and identify appropriate interventions to manage risk.
Provide in-patient to community"transition.
In home crisis support, outreach and engagement
Same day/next day services to clinic clients with urgent needs
Crisis management/Stabilization to any client in need
Assist with recovery from mental health or substance use.
Provide evidence-based interventions to enhance motivation to engage in services and supports and reduce risky behaviors.
Provide individual, group, family, collateral sessions and crisis services/interventions
Be actively self-reflective & self-aware of how clinical and administrative practice impacts clients, families, and the communities we serve
Approach work assigned with an anti-racist lens and open to continuously learning and evolving
Liaise with families, schools, criminal justice, and other community agencies to coordinate care
Participate in clinical meetings, staff meetings, trainings, and supervision.
Open and willing to receive constructive feedback
Maintain documentation according to the standards and time frames established by the Jewish Board, regulatory agencies and/or funding sources
Maintain professional behaviors and ethical standards as established by licensing board, relevant professional association and the Jewish Board policies and procedures.
Other duties as assigned.

Attending case conferences as needed
Maintaining a steady and active caseload
Meeting the minimum expectations for kept visits
EDUCATIONAL/TRAINING REQUIRED (List all that is required to achieve this position):
Master's degree in counseling, creative arts, or social work from an accredited program AND
New York State License or limited permit in Mental Health Counseling (LMHC); Creative Arts Therapy (LCAT), Licensed Marriage and Family Therapist (LMFT)or social work (LMSW) required.
EXPERIENCE REQUIRED/LANGUAGE PREFERENCE:
Knowledge and experience with evidence-based approaches
Fluency in a second language is preferred.
At least one year of previous clinical experience
Experience with documenting in electronic health records and using Microsoft Office software
Experience with telemental health platforms a plus
COMPUTER SKILLS REQUIRED (List the computer skills needed):
Proficient with electronic health record; Microsoft Office (Word, PowerPoint, Excel, Outlook); comfortable and proficient using audio video platforms (i.e., Zoom).
VISUAL AND MANUAL DEXIERITY:
The candidate should be able to read paper and electronic documents and perform significant data entry into various computer programs
WORK ENVIRONMENT / PHYSICAL EFFORT
Hybrid work setting. Position entails a combination of in person and remote work. Allocation of time and locations to be developed in collaboration with Senior Program Director and is based on the needs of the programs and clients served. Travel required between program locations and other Jewish Board sites.
