TEMP TO PERM CASE MANAGER ROLE.
Transitional Housing Case Manager (Temp-to-Perm)
Location: Brooklyn, NY
Schedule: Monday–Friday, 9:00 AM – 5:00 PM
Type: Full-Time, Temp-to-Perm
Education Requirement: Bachelor's Degree (Required)
Position Overview:
We are seeking a dedicated and compassionate Case Manager to support individuals and families transitioning out of homelessness. The ideal candidate will have experience working with vulnerable populations and a strong understanding of housing resources, social services, and community-based supports.
Key Responsibilities:

Provide case management services to residents in transitional housing programs.

Conduct intake assessments, develop individualized service plans, and monitor client progress.

Assist clients with securing permanent housing, employment, and benefits.

Coordinate referrals to healthcare providers, mental health services, and other support agencies.

Maintain detailed and timely case documentation in accordance with agency standards.

Participate in team meetings and collaborate with community partners.
Requirements:

Bachelor’s Degree in Social Work, Human Services, Psychology, or a related field (required)

Previous experience in housing programs or with homeless populations preferred

Strong interpersonal and crisis intervention skills

Familiarity with NYC housing systems and available resources

Proficiency in case documentation and time management

Bilingual a plus
