Responsibilities:

  • Conduct monthly home visits and weekly telephone contact with a caseload of up to 20 clients.

Interact with family and/or significant others regarding care.

  • Initiate new applicant screenings for housing and assist in obtaining needed documentation.
  • Prepare benefits/entitlements applications e.g. Section 8, SSI/SSD, Medicaid, and food stamps.
  • Complete referrals and link residents with community resources.
  • Assist clients with financial record keeping. Maintain accurate client records regarding their funds, budgetary item entries and benefits received.
  • Collect monthly rent from clients and prepare vouchers.
  • Maintain written and verbal communication with client landlords to request repairs, annual lease updates and resolve any other housing issues.
  • Complete program documentation including: progress notes, annual service plans, audit tools, and HUD timesheets. Provide statistical data required by Agency policy and program regulations.
  • Assess an emergency/crisis situation and effectively execute appropriate action to assist the client. Update supervisor regarding changes in client status.

Requirements:

  • High School diploma or GED; Bachelors degree preferred in Sociology or Psychology.
  • Two years of experience working with clients with mental health diagnoses OR a degree in Psychology or Sociology.
  • A valid NYS Drivers License and ability to travel locally across Long Island to meet with clients.
  • Ability to manage crisis situations during non-scheduled work hours.

Schedule: Full time - 35 hours per week Monday Friday 9 AM - 5 PM

Rate of Pay: $41,860 annual gross

This rate of pay serves as a good faith estimate. Actual pay is based on a number of factors which may include a candidates qualifications, skills, competencies, experience and work location. The salary range or rate listed does not include any differential pay or other forms of compensation, such as the value of benefits, which may be applicable to this position.
